| 摘要: |
| 目的 观察基于“经筋理论”针刺联合易筋经“九鬼拔马刀势”训练对颈源性头痛（CEH）患者的临床疗效。方法 收集2022年1月至2023年12月在湖南中医药大学第二附属医院诊疗的90例CEH患者，随机分为联合组（n=30）、针刺组（n=29）、功法组（n=31）。针刺组每3天针刺1次，共3次；功法组每天训练1次，共7次；联合组采用针刺加功法治疗，方法及疗程同针刺组和功法组。比较3组患者治疗前后视觉模拟评分（VAS）、关节活动度（ROM）评分、颈椎功能障碍指数（NDI）评分、血清白细胞介素-6（IL-6）水平、颈部肌肉弹性（杨氏模量值），并评定治疗后临床疗效。结果 治疗后，3组患者VAS评分、ROM评分、NDI评分、血清IL-6水平以及颈部肌肉杨氏模量值均较治疗前降低（P＜0.01）；且联合组患者VAS评分、ROM评分、NDI评分、血清IL-6水平均低于针刺组、功法组（P＜0.05，P＜0.01），而3组患者颈部肌肉杨氏模量值比较差异无统计学意义（P＞0.05）。联合组患者临床总有效率高于针刺组、功法组（P＜0.05，P＜0.01）。结论 基于“经筋理论”针刺联合易筋经“九鬼拔马刀势”训练治疗CEH疗效确切，能缓解患者疼痛程度，改善颈椎活动度、颈椎功能和肌肉弹性，降低血清IL-6水平，值得临床进一步推广应用。 |

| Abstract: |
| Objective To observe the clinical efficacy of acupuncture based on the "Sinew Theory" combined with the "Nine Fingers Drawing the Saber" exercise from Yi Jin Jing (a traditional Chinese Daoyin exercise for tendon-muscle strengthening and health maintenance) in patients with cervicogenic headache (CEH). Methods Ninety CEH patients diagnosed and treated at the Second Hospital of Hunan University of Chinese Medicine from January 2022 to December 2023 were enrolled and randomly assigned to a combination group (n=30), an acupuncture group (n=29), and an exercise group (n=31). The acupuncture group received acupuncture once every three days for a total of three sessions; the exercise group performed the training once daily for a total of seven sessions; the combination group received both acupuncture and exercise treatments, with methods and treatment courses identical to those of the respective groups. The visual analog scale (VAS) score, range of motion (ROM) score, neck disability index (NDI) score, serum interleukin-6 (IL-6) level, and neck muscle elasticity (Young's modulus) were compared before and after treatment among the three groups. The clinical efficacy after treatment was also evaluated. Results After treatment, the VAS, ROM, and NDI scores, serum IL-6 levels, and Young's modulus values of the neck muscles in all three groups were lower than those before treatment (P<0.01). Furthermore, the VAS, ROM, and NDI scores, and serum IL-6 levels in the combination group were lower than those in the acupuncture and exercise groups (P<0.05, P<0.01). However, there was no statistically significant difference in the Young's modulus values of the neck muscles among the three groups (P>0.05). The overall clinical effective rate in the combination group was higher than that in the acupuncture and exercise groups (P<0.05, P<0.01). Conclusion Acupuncture based on the "Sinew Theory" combined with "nine fingers drawing the saber" exercise from Yi Jin Jing has definite efficacy in treating CEH. It can alleviate pain intensity, improve neck ROM and function, enhance muscle elasticity, and reduce serum IL-6 level. It is worthy of further clinical promotion and application. |
