| 主观性耳鸣是耳鼻喉科常见的疑难杂症之一，中医整体调治具有优势。全国名中医袁长津教授对于该病的治疗积累了丰厚经验，从肝、脾、心三脏失和探讨其病机：肝失疏泄，气机不畅，布散逆乱无序为发病之始；脾失运化，酿痰生湿，清阳不升为病进之枢；心血不足，神失所养，脉道瘀涩不利为迁延之机。针对脏腑病机提出和中调枢以升清阳、清肝泻火以平少阳、健脾宁心以养气血、活血利水以通耳窍四大治法，并以补中益气汤为基础方，以方测证，合方加减，诠释辨证论治精要，临床疗效确切。并附验案一则以飨同道。 |

| Subjective tinnitus is one of the common and challenging disorders in otolaryngology, for which holistic TCM treatment demonstrates distinct advantages. Professor YUAN Changjin, a National Famous TCM Physician, has accumulated extensive experience in managing this condition. He explores its pathogenesis from the perspective of disharmony among the liver, spleen, and heart: failure of liver in governing the free flow of qi induces unsmooth qi movement, leading to chaotic distribution as the initial pathogenic factor; dysfunction of spleen in transportation and transformation causes phlegm-dampness accumulation and impairs the ascent of clear yang, serving as the pivotal mechanism for disease progression; and insufficient heart blood results in impaired nourishment of the spirit and obstructed collateral flow, contributing to chronicity. Based on the pathogenesis of zang-fu organs, he proposes four key therapeutic strategies: harmonizing the middle jiao and regulating the pivot to elevate clear yang, clearing liver fire to pacify the Shaoyang meridian, strengthening the spleen and calming the heart to nourish qi and blood, and promoting blood circulation and draining dampness to unblock the auditory orifices. Using Buzhong Yiqi Decoction as the foundational formula, he verifies patterns through formula efficacy, combines and modifies formulas accordingly, and elucidates the essence of treatment based on pattern differentiation, with proven clinical efficacy. A verified medical record is included for reference among peers. |
