| 大肠癌的发生率和死亡率在全球范围内持续上升，已严重威胁人类健康。脂质代谢重编程作为大肠癌的重要生物学特征之一，贯穿病程始终，与其发生发展和转归密切相关。结合中医学“脾气散精”理论，揭示“脾失散精-脂浊内生-癌浊结聚”的病机演变过程，该过程亦为大肠癌脂质代谢重编程发生发展的关键病机。基于此病机，确立助脾散精、降脂化浊、祛痰消积三大治法，共奏复其本、清其源、攻其结之效，使脾气充盈、健运有司，精微得以布散，浊邪无以内生，癌瘤难以成形。此策略有助于恢复“脾气散精”的正常功能，为中医药治疗大肠癌提供新思路。 |

| Pathogenetic progression and treatment strategies for colorectal cancer based on "spleen qi disperses essence" and lipid metabolic reprogramming |

| Abstract: |
| The incidence and mortality of colorectal cancer continue to rise worldwide, posing a serious threat to human health. Lipid metabolic reprogramming, an important biological characteristic of colorectal cancer, is involved throughout the disease course and is closely associated with the onset, progression, and outcome of colorectal cancer. Based on the TCM theory that "spleen qi disperses essence", this article reveals a pathomechanistic progression from "failure of the spleen to disperse essence-internal generation of lipid turbidity-aggregation of cancer turbidity", which also represents the key pathomechanism underlying the occurrence and development of lipid metabolic reprogramming in colorectal cancer. Based on this pathomechanism, three treatment methods are established: assisting the spleen to disperse essence, lowering lipid levels and resolving turbidity, and dispelling phlegm and eliminating accumulation, thereby restoring the root, clearing the source, and eliminating the accumulation. These approaches replenish spleen qi, restore its healthy transportation, facilitate the distribution of essence, prevent the internal generation of pathogenic turbidity, and impede tumor formation. This strategy helps restore the normal function of spleen qi in dispersing essence and provides a new approach for treating colorectal cancer with Chinese medicine. |
