| 摘要: |
| 痉挛性斜颈（ST）是成人最常见的局限性肌张力障碍。西医常规治疗虽有一定疗效，但整体获益有限。傅健认为，ST的核心病机为肝失疏泄，继而出现气滞血瘀、湿热内蕴，病久兼虚，诸变引动内风，窜扰颈项，致筋脉失控而发病。傅健临证主张“调肝为本，通利为要，以和为贵”的治疗原则，主方选用经验方首珍汤加减。基于该学术思想治疗ST对改善颈部异常姿势及运动障碍具有良好的临床疗效，附验案一则加以阐明，以期为进一步探索ST的临证治疗提供初步的个案依据与启发思路。 |

| Abstract: |
| Spasmodic torticollis (ST) is the most common focal dystonia in adults. Conventional Western medical treatment has certain efficacy, but its overall clinical effectiveness remains limited. FU Jian believes that the core pathogenesis of ST is the liver failing to maintain the free flow of qi, which subsequently leads to qi stagnation and blood stasis, internal accumulation of damp heat, and, in prolonged cases, deficiency. These various pathological changes stir up internal wind, which disturbs the neck, causing the sinews and vessels to lose control, thus triggering the onset of the disease. In clinical practice, FU Jian advocates the therapeutic principle of "regulating the liver as the root, promoting unimpeded flow as the key, and valuing harmony as the priority," and adopts the modified Shouzhen Decoction (an empirical formula) as the main formula. Based on this academic thought, the treatment of ST demonstrates good clinical efficacy in rectifying abnormal neck posture and alleviating movement disorders. A verified medical record is attached for illustration, with the aim of providing preliminary case-based evidence and inspiring ideas for further exploration of clinical treatment of ST. |
