清解止抽汤治疗风邪犯肺引动肝风型儿童暂时性抽动障碍的临床研究

清解止抽汤治疗风邪犯肺引动肝风型儿童暂时性抽动障碍的临床研究

摘要:

目的观察清解止抽汤治疗风邪犯肺引动肝风型儿童暂时性抽动障碍（PTD）的临床疗效及对免疫炎症指标的影响。方法选择2024年7月至2025年9月湖南中医药大学第一附属医院儿科门诊就诊的54例风邪犯肺引动肝风型PTD患儿为研究对象。采用随机数字表法分为观察组28例，对照组26例。对照组口服氟哌啶醇片治疗，观察组口服清解止抽汤治疗，两组均连续治疗2周。治疗后，比较两组耶鲁综合抽动严重程度量表（YGTSS）评分、中医证候积分、临床疗效、白细胞介素（IL）-6、IL-2、IL-10、肿瘤坏死因子-α（TNF-α）、高香草酸（HVA）水平，T淋巴细胞亚群（CD3⁺、CD4⁺、CD8⁺、CD4⁺/CD8⁺）水平，并记录不良反应。结果与治疗前比较，两组治疗后YGTSS评分、中医证候积分、IL-6、IL-2、TNF-α、CD8⁺水平均降低（P<0.05），IL-10、HVA、CD3⁺、CD4⁺、CD4⁺/CD8⁺水平均升高（P<0.05）。与对照组比较，观察组YGTSS评分、中医证候积分、IL-6、IL-2、TNF-α、CD8⁺水平均降低（P<0.05，P<0.01），IL-10、HVA、CD3⁺、CD4⁺、CD4⁺/CD8⁺水平均升高（P＜0.05）。观察组总有效率为92.86%，对照组为76.92%，两组比较差异无统计学意义（P＞0.05），两组患儿均未出现严重不良反应。结论清解止抽汤可能通过下调IL-6、IL-2、TNF-α、CD8⁺水平，上调IL-10、HVA、CD3⁺、CD4⁺、CD4⁺/CD8⁺水平，调控免疫炎症平衡，从而改善患儿临床症状。

Clinical study of Qingjie Zhichou Decoction for childhood provisional tic disorder of wind invading the lung and stirring liver wind pattern

Abstract:

Objective To observe the clinical efficacy of Qingjie Zhichou Decoction (QJZCD) in treating children with provisional tic disorder (PTD) of wind invading the lung and stirring liver wind pattern and its effect on immune-inflammatory markers. Methods A total of 54 children with PTD of wind invading the lung and stirring liver wind pattern, who attended the Pediatric Outpatient Department at the First Hospital of Hunan University of Chinese Medicine from July 2024 to September 2025, were enrolled as research subjects. They were divided into an observation group (28 cases) and a control group (26 cases) using a random number table. The control group was treated with oral haloperidol tablets, while the observation group was treated with oral QJZCD. Both groups were treated for 2 consecutive weeks. After treatment, the Yale Global Tic Severity Scale (YGTSS) score, TCM pattern scores, clinical efficacy, levels of interleukin (IL)-6, IL-2, IL-10, tumor necrosis factor-α (TNF-α), homovanillic acid (HVA), as well as T lymphocyte subsets (CD3⁺, CD4⁺, CD8⁺, CD4⁺/CD8⁺), and adverse reactions were compared between the two groups. Results Compared with the baseline, YGTSS scores, TCM pattern scores, as well as levels of IL-6, IL-2, TNF-α, and CD8⁺ were reduced (P<0.05), whereas levels of IL-10, HVA, CD3⁺, CD4⁺, and CD4⁺/CD8⁺ were elevated (P<0.05) in both groups after treatment. Compared with the control group, the observation group showed reduced YGTSS scores and TCM pattern scores, lower levels of IL-6, IL-2, TNF-α, and CD8⁺ (P<0.05, P<0.01), and higher levels of IL-10, HVA, CD3⁺, CD4⁺, and CD4⁺/CD8⁺ (P＜0.05). The total effective rate was 92.86% in the observation group and 76.92% in the control group, with no statistically significant difference between the two groups (P＞0.05). No severe adverse reactions occurred in either group. Conclusion QJZCD may alleviate clinical symptoms of children with PTD by down-regulating the levels of IL-6, IL-2, TNF-α, and CD8⁺, and up-regulating the levels of IL-10, HVA, CD3⁺, CD4⁺, and CD4⁺/CD8⁺, thereby regulating the balance of cellular immunity and inflammation.
