| 老年失眠症是困扰老年群体的常见健康问题，其会导致认知、免疫等功能下降，并增加冠心病、高血压等疾病发生的风险。中医诊治失眠素有从心、肝、脾、肾立论的传统，却较少关注肺脏在睡眠调节中的特殊作用。本文立足《黄帝内经》“肺主治节”理论，系统阐释肺脏通过治理气机、调控营卫、辅心行血等功能对人体生理节律的重要影响，指出老年肺气衰退、治节失常是引发失眠的关键病机。基于此，提出以“益肺气、调宣肃、复治节”为核心的治疗原则，并分别探讨宣痹汤调畅气机、桂枝加厚朴杏子汤调和营卫、补肺汤合当归补血汤益气养血的临证应用。并附验案一则以佐证。 |

| Treatment of senile insomnia based on the theory of "the lung governing management and regulation" |

| Abstract: |
| Senile insomnia(SI) is a common health problem affecting the elderly population, which may lead to declines in cognitive and immune functions and increase the risk of diseases such as coronary heart disease and hypertension. In Chinese medicine, the diagnosis and treatment of insomnia have traditionally been approached from the perspectives of the heart, liver,spleen, and kidney, while relatively little attention has been paid to the specific role of the lung in sleep regulation. Based on the theory of "the lung governing management and regulation" as described in the Huang Di Nei Jing(Huangdi's Classic of Medicine),this paper systematically elucidates the crucial roles of the lung in maintaining physiological rhythms through regulating qi movement, coordinating Ying-nutrient and Wei-defensive qi, and assisting the heart in blood circulation. It is proposed that age-related lung qi deficiency and dysfunction of its regulatory function constitute a key pathogenesis of insomnia in the elderly.Accordingly, core therapeutic principles are formulated as tonifying lung qi, regulating diffusion and depuration, and restoring management and regulation. Clinical applications are further discussed, including the use of Xuanbi Decoction to regulate qi movement, modified Guizhi combined with Houpo Xingzi Decoction to harmonize Ying-nutrient and Wei-defensive qi, and Bufei Decoction combined with Danggui Buxue Decoction to supplement qi and nourish blood. A verified medical record is presented to support the proposed approach. |
