推拿治疗坐骨神经损伤作用机制的研究进展

摘要:
    坐骨神经损伤是临床常见的周围神经损伤性疾病,常引发感觉运动障碍及慢性疼痛,严重影响患者的生活质量。目前临床治疗以手术修复、药物干预及物理康复为主,但存在神经再生缓慢、功能恢复不完全等局限。中医学认为,该病核心病机为气血瘀滞、筋脉失养,而推拿作为中医特色非药物疗法,可通过手法刺激疏通经络、调和气血、濡养筋脉,契合病机特点,在促进神经修复、缓解疼痛等方面展现出独特优势。本文系统综述近十年来推拿干预坐骨神经损伤的研究进展,从促进神经再生与修复、改善局部血流灌注、减轻神经炎症反应、抑制小胶质细胞活化、减少疼痛介质表达、调节突触可塑性、延缓肌萎缩及调节神经肌肉信号传导方面,探讨推拿治疗坐骨神经损伤的作用机制,为其临床规范化应用及后续科研提供理论支撑。

Research progress on the mechanism of action of tuina in treating sciatic nerve injury

Abstract:
    Sciatic nerve injury is a clinically common peripheral nerve injury disease, often leading to sensorimotor dysfunction and chronic pain that severely impairs patients' quality of life. Current clinical treatments primarily involve surgical repair, pharmacological interventions, and physical rehabilitation. However, limitations such as slow nerve regeneration and incomplete functional recovery persist. According to Chinese medicine, the core pathogenesis of this disease involves qi stagnation and blood stasis as well as malnutrition of tendons and meridians. As a distinctive non-pharmacological therapy in Chinese medicine, tuina employs manual techniques to stimulate and unblock meridians and collaterals, regulate and harmonize qi and blood, as well as nourish tendons and meridians, aligning well with the pathological characteristics. It demonstrates unique advantages in promoting nerve repair and alleviating pain. This article systematically reviews the research progress over the past decade on tuina intervention for sciatic nerve injury. It explores the mechanism of action of tuina therapy from multiple perspectives, including promoting nerve regeneration and repair, improving local blood perfusion, alleviating neuroinflammation, inhibiting microglial activation, reducing pain mediator expression, modulating synaptic plasticity, delaying muscle atrophy, and regulating neuromuscular signal transduction. This review aims to provide theoretical support for its standardized clinical application and future scientific research.
