| 摘要: |
| 慢性胃炎“炎癌转化”过程涉及代谢重编程与免疫机制的互相干扰及整体失衡。基于“火与元气不两立”理论，并结合现代研究成果，认为“气火失衡”与“代谢-免疫失衡”具有内在相通性。由脾胃虚损所致的元气虚损是“炎癌转化”发生的病理基础，元气虚损、阴火上乘所致的“气火失衡”是驱动“代谢-免疫失衡”的核心病机，进而衍生痰瘀等病理产物胶结，共同推动疾病演进。根据病机演变进程提出以补元气、泻阴火为总则的分期论治策略：初始期主张调补中焦、补益元气，进展期主张清泻阴火、固本扶正，癌变期主张益气养阴、解毒固本，为慢性胃炎“炎癌转化”的基础研究及临床治疗提供参考。 |

| Abstract: |
| The process of inflammation-cancer transformation in chronic gastritis involves mutual interference and systemic imbalance between metabolic reprogramming and immune mechanisms. Based on the theory of "yin fire and Yuan-primordial qi are intrinsically opposed," combining modern research findings, it is believed that "qi-fire imbalance" shares an intrinsic connection with "metabolism-immune imbalance.” The deficiency of Yuan-primordial qi resulting from the deficiency of spleen and stomach constitutes the pathological basis for the occurrence of inflammation-cancer transformation, and the qi-fire imbalance caused by deficient Yuan-primordial qi and hyperactive yin fire serves as the central pathogenesis driving metabolism-immune imbalance. This further leads to the accumulation of pathological products such as phlegm and blood stasis, thereby accelerating disease progression. A staged therapeutic approach centered on the general principle of "tonifying Yuan-primordial qi and clearing yin fire" is proposed according to the evolution of pathogenesis. In the initial stage, treatment focuses on strengthening the middle jiao and tonifying Yuan-primordial qi. In the progressive stage, the emphasis shifts to clearing and reducing yin fire, reinforcing healthy qi to strengthen the foundation. In the cancer transformation stage, intervention requires reinforcing qi, nourishing yin, removing toxins, and strengthening the foundation. This paper offers a reference for basic research and clinical treatment of inflammation-cancer transformation in chronic gastritis. |
