基于“阳微阴弦”理论探讨血管平滑肌细胞增殖与迁移在动脉粥样硬化中的作用

摘要:
    动脉粥样硬化(AS)是一种以动脉内脂质沉积和炎症细胞积累为特征的复杂血管疾病,常引发心脑血管不良事件。近年来,血管平滑肌细胞(VSMC)的表型转换,特别是从收缩型向合成型的转变,被认为是AS进展的关键机制。收缩型VSMC主要维持血管张力和结构稳定性,而合成型VSMC则表现出分泌、增殖和迁移能力的增强,促进病理性血管重塑。在中医理论中,收缩型VSMC的功能特性可与“阳”相类比,而合成型VSMC的物质合成功能则与“阴”相关联,二者的动态转化与“阳微阴弦”病机理论具有一定契合性。基于此,本研究结合中医理论与现代细胞分子机制,探讨VSMC表型转换在AS中的作用机制,并梳理了中医治法治则,以及中药单体、复方在调控VSMC表型转换、抑制异常增殖与迁移中的作用机制及靶点,旨在为AS的临床防治和基础研究提供新的思路,并从中医视角深化对VSMC在AS中的作用机制的理解。

The role of vascular smooth muscle cell proliferation and migration in atherosclerosis based on the theory of "yang deficiency and yin excess"

Abstract:
    Atherosclerosis (AS) is a complex vascular disease characterized by lipid deposition and the accumulation of inflammatory cells within the arterial wall, often leading to adverse cardiovascular and cerebrovascular events. In recent years, vascular smooth muscle cell (VSMC) phenotypic switching, particularly the transition from the contractile to the synthetic phenotype, has been recognized as a key mechanism in AS progression. Contractile VSMCs primarily maintain vascular tone and structural stability, whereas synthetic VSMCs exhibit enhanced secretory, proliferative, and migratory capacities, thereby promoting pathological vascular remodeling. In TCM theory, the functional characteristics of contractile VSMCs can be analogized to "yang", while the biosynthetic activity of synthetic VSMCs corresponds to "yin", The dynamic transformation between these phenotypes corresponds, to a certain extent, with the TCM pathogenetic theory of "yang deficiency and yin excess". Based on this, the study integrates the TCM theory with modern cellular and molecular mechanisms to investigate the role of VSMC phenotypic switching in the AS pathogenesis. Additionally, it systematically reviews TCM therapeutic principles and methods, and the mechanisms and molecular targets of various single herbal compounds and formulas in regulating VSMC phenotypic switching and inhibiting abnormal proliferation and migration. This study aims to provide new insights for both the clinical management and fundamental research of AS while offering a unique perspective on the role of VSMCs in AS from the standpoint of TCM.
