| 基于温肾活血解毒法探讨肾气丸联合西黄丸治疗去势抵抗性前列腺癌 |

| 摘要: |
| 前列腺癌多数患者经西医手术或药物去势治疗后，最终转化为去势抵抗性前列腺癌（castration resistant prostate cancer，CRPC）伴骨转移风险增加，而中医药治疗目前尚缺乏统一规范的诊疗方案。基于中医理论，探讨CRPC病机及治法特点，认为CRPC基本病机为肾阳亏虚、血瘀毒结，治法核心为温肾活血解毒。作为温补肾阳和活血解毒的经典代表方，肾气丸可温阳利水、补肾壮骨、散寒止痛，西黄丸可活血消癥、清解毒邪、行气止痛，两方合用总起温肾活血解毒之效，符合CRPC中医治法要求。结合现代研究进展和方药分析，认为肾气丸联合西黄丸可能会在改善CRPC下尿路梗阻、骨转移疼痛及体能下降等症状方面具有一定治疗作用。因此，提出肾气丸合西黄丸运用于治疗CRPC，以期为中医药临床治疗提供新思路。 |

| Most patients with prostate cancer will eventually transform into castration-resistant prostate cancer (CRPC) with increased risk of bone metastasis after western surgery or drug castration treatment. However, there is still a lack of unified and standardized diagnosis and treatment plan for traditional Chinese medicine (TCM) treatment. Based on the theory of TCM, this paper discusses the pathogenesis and treatment characteristics of CRPC, and holds that the basic pathogenesis of CRPC is kidney Yang deficiency and blood stasis and toxin aggregation, and the core of treatment is warming kidney, activating blood circulation and detoxification. As the classic representative prescription of warming and tonifying kidney Yang and promoting blood circulation and detoxification, Shenqi Pill can warm Yang and benefit water, tonify kidney and strengthen bone, disperse cold and relieve pain, Xihuang Pill can promote blood circulation and eliminate agglomeration, clear and detoxify evil, activate Qi and relieve pain. The combined use of the two prescriptions has the effect of warming kidney and activating blood circulation and detoxification, which meets the requirements of CRPC's TCM treatment. Combined with the modern research progress and prescription analysis, it is believed that Shenqi Pill combined with Xihuang Pill may have a certain therapeutic effect on improving the symptoms of CRPC lower urinary tract obstruction, bone metastasis pain and physical decline. Therefore, Shenqi Pill and Xihuang Pill are proposed to be used in the treatment of CRPC, in order to provide a new idea for TCM clinical treatment. |
