中药穴位敷贴联合常规抗结核疗法对耐药肺结核患者免疫功能、呼吸功能及临床疗效的影响

摘要:
    目的 观察中药穴位敷贴联合常规抗结核疗法治疗耐药肺结核患者的临床疗效,以及对患者免疫功能及呼吸功能的影响。方法 将收治的104例耐药肺结核患者按照随机数字表法分为观察组和对照组(各52例),对照组采用常规抗结核疗法治疗,观察组在对照组的基础上采用中药穴位敷贴治疗。于治疗前后检测比较两组患者外周血T淋巴细胞亚群CD3+、CD4+、CD8+水平和血清免疫球蛋白IgG、IgA、IgM水平,并计算出CD4+/CD8+,对比两组患者肺功能相关指标[包括用力肺活量(forced vital capacity,FVC)、第1秒用力呼吸量(forced expiratory volume in one second,FEV1)、第1秒用力呼吸容积占用力肺活量百分比(FEV1/FVC)],并于治疗后进行病灶情况和临床疗效评定。结果 (1)治疗后,两组患者的CD3+、CD4+、IgG、IgA、IgM水平及CD4+/CD8+均较治疗前升高(P<0.05),观察组CD8+水平较治疗前下降(P<0.05),且观察组CD3+、CD4+、IgG、IgA、IgM水平及CD4+/CD8+均高于对照组(P<0.05),CD8+水平低于对照组(P<0.05);(2)治疗后,两组患者FVC、FEV1、FEV1/FVC均较治疗前升高(P<0.05),且观察组高于对照组(P<0.05);(3)治疗后,观察组吸收率和总有效率均为96.15%,高于对照组的82.69%和80.77%(P<0.05)。结论 中药穴位敷贴联合常规抗结核疗法治疗耐药肺结核患者可提高机体免疫功能和改善肺功能,临床疗效优于单纯常规抗结核疗法,值得推广。

Effects of Acupoint Application of Chinese Materia Medica Combined with Conventional Anti-Tuberculosis Therapy on Immune Function, Respiratory Function and Clinical Efficacy of Drug-Resistant Pulmonary Tuberculosis Patients

Abstract:
    Objective To observe the clinical efficacy of acupoint application of Chinese materia medica combined with conventional anti-tuberculosis therapy in the treatment of drug-resistant tuberculosis patients, as well as the influence on the immune function and respiratory function of patients. Methods A total of 104 patients with drug-resistant pulmonary tuberculosis were divided into an observation group and a control group (52 cases in each group) according to random number table. The control group was treated with conventional anti-tuberculosis therapy, while the observation group was treated with acupoint application of Chinese materia medica on the basis of the control group. The levels of peripheral blood T lymphocyte subsets CD3+, CD4+, CD8+ and serum immunoglobulin IgG, IgA and IgM were detected and compared before and after treatment, and the levels of CD4+/CD8+ were calculated. The lung function related indexes[including forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC] of the 2 groups were compared. The lesions and clinical efficacy were evaluated after treatment. Results (1) After treatment, the levels of CD3+, CD4+, IgG, IgA, IgM and CD4+/CD8+ in the 2 groups were increased than those before treatment (P<0.05). The levels of CD8+ in the observation group were decreased than those before treatment (P<0.05). The levels of CD3+, CD4+, IgG, IgA, IgM and CD4+/CD8+ in the observation group were higher than those in the control group (P<0.05), and the levels of CD8+ in the observation group were lower than those in the control group (P<0.05); (2) After treatment, the levels FVC, FEV1, FEV1/FVC were increased than before treatment (P<0.05), and the observation group was higher than the control group (P<0.05); (3) After treatment, the absorption rate and total effective rate of the observation group were 96.15%, higher than 82.69% and 80.77% of the control group (P<0.05). Conclusion Acupoint application of Chinese materia medica combined with conventional anti-tuberculosis therapy can improve the immune function and lung function of drug-resistant tuberculosis patients, and the clinical efficacy is better than that of conventional anti-tuberculosis therapy alone, which is worth promoting.
