| 摘要: |
| 目的 探讨体外冲击波联合滑膜炎颗粒治疗湿热痹阻型膝骨关节炎(KOA)的临床疗效。方法 选取2021年9月至2022年8月在石家庄市中医院就诊的100例KOA患者,随机分为两组,每组50例。对照组口服滑膜炎颗粒,观察组口服滑膜炎颗粒+体外冲击波,治疗4周。观察并记录两组患者肌肉骨骼功能评分(SMFA)、股四头肌平均功率(AP)及峰力矩(PT)、步态时空及对称性参数、血清免疫炎症因子水平数据,进行比较分析。结果 两组治疗后,观察组SMFA较治疗前降低(P<0.05),股四头肌AP、PT较治疗前均升高(P<0.05);观察组步态参数优于对照组(P<0.05)。两组治疗后血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β、IL-6、IL-8及干扰素-γ(IFN-γ)较治疗前均降低(P<0.05)。T细胞免疫球蛋白黏蛋白3(Tim-3)、程序性死亡受体1(PD-1)水平均高于治疗前(P<0.05),且观察组优于对照组(P<0.05)。结论 体外冲击波联合滑膜炎颗粒可治疗湿热痹阻型KOA,可减轻患者自身免疫反应及炎症反应,增强其股四头肌肌力,使关节稳定,恢复关节平衡能力。 |

| Effects of extracorporeal shock wave therapy combined with Synovitis Granules on muscle function and gait characteristics in patients with knee osteoarthritis |

| Objective To explore the clinical effects of extracorporeal shock wave therapy (ESWT) combined with Synovitis Granules in the treatment of damp-heat obstruction type knee osteoarthritis (KOA). Methods A total of 100 KOA patients who visited Shijiazhuang Traditional Chinese Medicine Hospital from September 2021 to August 2022 were randomly divided into two groups, with 50 cases in each group. The control group was treated with oral Synovitis Granules, while the observation group with oral Synovitis Granules combined with ESWT. The clinical observation lasted for 4 weeks. The score of Short Musculoskeletal Function Assessment (SMFA), average power (AP) and peak torque (PT) of the quadriceps femoris, gait spatiotemporal and symmetry parameters, and serum immune and inflammatory factor levels were observed and recorded for both groups, and followed by comparative analysis. Results After treatment in both groups, the observation group showed a decrease in SMFA score, while an increase in AP and PT of the quadriceps femoris compared with those before treatment (P<0.05). The improvements of gait parameters in the observation group was better than that in the control group (P<0.05). After treatment, serum levels of TNF-α, IL-1β, IL-6, IL-8, and IFN-γ in both groups decreased compared with those before treatment (P<0.05); however, the levels of Tim-3 and PD-1 were higher than those before treatment (P<0.05), with the observation group showing better results than the control group (P<0.05). Conclusion The combination of ESWT and Synovitis Granules in the treatment of damp-heat obstruction type KOA can alleviate the patient's autoimmune and inflammatory responses, enhance the muscle strength of the quadriceps femoris, stabilize the joint, and restore its balance ability. |
