| 摘要: |
| 抽动障碍是起病于儿童及青少年时期的神经精神障碍,临床表现为重复、突然和不自主的动作或发声,常伴有精神病理及行为方面的共病,与中医“形神合一”理论中“形”与“神”双重失调的特征相符。本文立足于“形神合一”理论,首先梳理该理论渊源及其在儿科心身疾病诊疗中的指导意义;继而从肝亢风动、脾虚痰聚、肝郁脾虚3个维度阐释儿童抽动障碍“形神互损”的病机本质;在此基础上,结合现代神经生化、神经免疫及脑功能研究,提出耳穴疗法作为“形神共治”干预窗口的理论依据;进而针对肝亢风动、脾虚痰聚、肝郁脾虚三证,分别提出对应的耳穴疗法,并探讨其在神经递质调节、炎症抑制及下丘脑-垂体-肾上腺(HPA)轴调控方面的潜在作用机制。本文旨在为耳穴疗法治疗儿童抽动障碍提供系统的理论支撑与机制阐释,推动中医外治疗法在儿科心身疾病中的深化应用。 |

| Auricular acupoint therapy for tic disorders in children from the theory of “unity of body and mind” |

| Abstract: |
| Tic disorders are childhood and adolescent-onset neuropsychiatric disorders characterized by repetitive, sudden, and involuntary movements or vocalizations, and are often accompanied by psychopathological and behavioral comorbidities, which correspond to the dual dysregulation of the body and mind described in the TCM theory of "unity of body and mind". Based on the theory, this article first reviews the origin of the theory and its guiding significance in the diagnosis and treatment of pediatric psychosomatic disorders; subsequently, it elucidates the pathogenesis of "mutual impairment of body and mind" in tic disorders in children from three aspects: hyperactivity of the liver with wind stirring, spleen deficiency with phlegm accumulation, and liver qi stagnation with spleen deficiency; on this basis, in conjunction with modern research on neurobiochemistry, neuroimmunology, and brain function, it proposes the theoretical basis for auricular acupoint therapy as an intervention for treating both body and mind; furthermore, it proposes corresponding auricular acupoint therapies for the three patterns of hyperactivity of the liver with wind stirring, spleen deficiency with phlegm accumulation, and liver qi stagnation with spleen deficiency, respectively, and explores their potential mechanisms involving neurotransmitter regulation, suppression of inflammation, and regulation of the hypothalamic-pituitary-adrenal (HPA) axis. This article aims to provide systematic theoretical support and mechanistic explanations for auricular acupoint therapy in the treatment of tic disorders in children, and to promote the further application of external therapies of Chinese medicine in pediatric psychosomatic disorders. |
