| 卒中后痉挛(PSS)是脑卒中后常见的运动功能障碍,严重影响患者运动功能恢复与生活质量。其病理机制涉及中枢抑制能力下降、脊髓环路重构等多重因素,临床治疗难度较大。针刺治疗PSS临床疗效确切,脑源性神经营养因子(BDNF)及其受体酪氨酸激酶受体B(TrkB)介导的BDNF/TrkB信号通路在调控突触可塑性、维持神经元存活中发挥重要作用。本文以BDNF/TrkB信号通路为切入点,系统梳理针刺通过调控该通路改善PSS的作用机制,以期为针刺治疗PSS的机制研究与临床应用提供理论依据。 |

| Abstract: |
| Post-stroke spasticity (PSS) is a common motor dysfunction following stroke that severely impairs motor recovery and quality of life. Its pathological mechanisms involve multiple factors, including reduced central inhibitory control and remodeling of spinal neural circuits, bringing great difficulties to clinical treatment. Acupuncture has demonstrated favorable clinical efficacy in treating PSS. The signaling pathway mediated by brain-derived neurotrophic factor (BDNF) and its receptor, tropomyosin receptor kinase B (TrkB), plays an important role in regulating synaptic plasticity and promoting neuronal survival. Focusing on the BDNF/TrkB signaling pathway, this paper systematically summarizes the potential mechanism of action by which acupuncture alleviates PSS through modulation of this pathway, with the aim of providing a theoretical basis for further mechanistic research and the clinical application of acupuncture in the management of PSS. |
