| 本文旨在突破传统“风、火、痰、瘀、虚”的中风病机论述框架,以《黄帝内经》“阳化气,阴成形”核心理论为根基,构建缺血性中风阴阳形气失衡的全维度病机模型,系统阐释本病从基础发病到急性期进展的完整病理链条。研究以“形气平衡”为核心,首先厘清脑府生理状态下“阳化气”与“阴成形”的互济协同机制,明确“火热伤络”是连接功能失衡与形质损伤的核心枢纽,同时阐明“阳化气不足生痰湿、阴成形太过生瘀浊”在本病发病中的基础作用。基于此,提出缺血性中风“调节形气、调和阴阳”的总治则,急性期以“清火以抑阳化气太过、护阴以复阴成形功能”为核心,兼顾化痰逐瘀、温阳化气以纠正基础失衡,为临床诊疗提供深层理论依据。 |

| Pathogenesis and treatment of ischemic stroke from the perspective of "yang transforming qi while yin constituting form" |

| Abstract: |
| This paper aims to transcend the traditional framework of stroke pathogenesis that relies on the concepts of "wind, fire, phlegm, stasis, and deficiency." Grounded in the core theory of "yang transforming qi while yin constituting form" from the Huang Di Nei Jing (Huangdi's Classic of Medicine), it constructs a comprehensive pathogenesis model of yin-yang and form-qi imbalance for ischemic stroke (IS), systematically elucidating the complete pathological chain from the initial onset to the progression of the acute phase. Centered on the "form-qi balance," the paper first clarifies the mutual assistance and synergistic mechanism between "yang transforming qi" and "yin constituting form" under physiological conditions of the brain. It then identifies that "fire-heat damaging the collaterals" serves as the core pivot linking functional imbalance and structural damage. Simultaneously, it elucidates the fundamental roles of "insufficient yang transformation of qi generating phlegm and dampness" and "hyperactivity of yin in constituting form leading to blood stasis and turbidity" in the pathogenesis of this disease. Based on this, the general therapeutic principle of "regulating form and qi, and harmonizing yin and yang" for IS is proposed. In the acute phase, the core approach is "clearing fire to restrain excessive yang transformation of qi, and protecting yin to restore its function in constituting form," while concurrently resolving phlegm and stasis, warming yang and transforming qi to correct the underlying imbalance. This provides a profound theoretical foundation for clinical diagnosis and treatment. |
