| 摘要: |
| 目的 观察电针次髎穴对脊髓损伤(SCI)大鼠神经源性肠道功能障碍(NBD)的改善作用,并探讨其与直肠组织G蛋白偶联胆汁酸受体5(TGR5)表达、肠道炎症及平滑肌电活动的相关性。方法 选取42只SPF级雌性SD大鼠,随机抽取10只作为假手术组,其余32只用于造模;采用改良Hassan-Shaker法建立T10节段脊髓全横断模型,最终造模成功大鼠22只,随机选取20只分为模型组与针刺组,每组10只。造模第19天起,针刺组大鼠双侧次髎穴给予疏密波电针干预(疏波10 Hz、密波50 Hz,20 min/次,1次/d,连续7 d),假手术组与模型组仅行同等时长捆绑固定处理。检测大鼠体质量、粪便含水率、肠道推进率;采用HE染色观察直肠组织病理形态;采用HPLC、ELISA、荧光探针法分别检测直肠组织5-羟色胺(5-HT)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及钙离子(Ca2+)浓度;采用免疫组织化学法检测直肠组织TGR5蛋白表达;记录直肠肌电图(EMG)振幅变化。结果 与假手术组比较,模型组大鼠体质量、粪便含水率、肠道推进率、直肠组织5-HT、Ca2+浓度、TGR5表达水平及直肠EMG振幅均降低(P<0.05),TNF-α、IL-6升高(P<0.05),直肠组织出现杯状细胞减少、炎症细胞浸润等病理损伤;与模型组比较,针刺组大鼠体质量、粪便含水率、肠道推进率、直肠组织5-HT、Ca2+浓度、TGR5表达水平及直肠EMG振幅均升高(P<0.05),TNF-α、IL-6均降低(P<0.05),直肠组织病理损伤明显减轻。结论 电针次髎穴可改善SCI大鼠NBD,增强直肠肌电活动、减轻肠道炎症反应并上调TGR5表达,其中电针改善NBD的机制与上调TGR5 表达密切相关。 |

| Effects of electroacupuncture at Ciliao (BL32) on rectal myoelectric activity and TGR5 expression in rats with spinal cord injury-induced neurogenic intestinal dysfunction |

| Objective To observe the therapeutic effects of electroacupuncture at Ciliao (BL32) on neurogenic bowel dysfunction (NBD) in rats with spinal cord injury (SCI), and to explore its correlation with the expression of Takeda G protein-coupled receptor 5 (TGR5), intestinal inflammation and smooth muscle myoelectric activity in rectal tissues. Methods A total of 42 SPF-grade female SD rats were selected, with 10 randomly assigned to the sham-operated group and the remaining 32 used for model establishment. A modified Hassan-Shaker method was adopted to establish the complete spinal cord transection model at the T10 vertebral body. Finally, 22 rats were successfully modeled, 20 of which were randomly divided into model group and acupuncture group, with 10 rats in each group. From the 19th day after modeling, rats in the acupuncture group received sparse-dense wave electroacupuncture intervention at bilateral Ciliao (BL32) (sparse wave: 10 Hz, dense wave: 50 Hz, 20 minutes per treatment, once a day for 7 consecutive days). Rats in the sham-operated group and model group were only bundled and fixed for the same duration. The body weight, fecal water content and intestinal propulsion rate of rats were measured. Hematoxylin-eosin (HE) staining was used to observe the pathological morphology of rectal tissues. High-performance liquid chromatography (HPLC), enzyme-linked immunosorbent assay (ELISA) and fluorescent probe technology were used to check the concentrations of 5-hydroxytryptamine (5-HT), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and calcium ion (Ca2+) in rectal tissues, respectively. Immunohistochemistry was performed to measure the expression of TGR5 protein in rectal tissues, and the amplitude variations of rectal electromyography (EMG) were recorded. Results Compared with the sham-operated group, the model group showed a decrease in body weight, fecal water content, intestinal propulsion rate, concentrations of 5-HT and Ca2+ in rectal tissues, TGR5 expression level and rectal EMG amplitude (P<0.05), an increase in TNF-α and IL-6 levels (P<0.05), along with pathological lesions including goblet cell reduction and inflammatory cell infiltration in rectal tissues. Compared with the model group, the acupuncture group showed elevated body weight, fecal water content, intestinal propulsion rate, concentrations of 5-HT and Ca2+ in rectal tissues, TGR5 expression level and rectal EMG amplitude (P<0.05), reduced TNF-α and IL-6 levels (P<0.05), and markedly alleviated pathological lesions in rectal tissues. Conclusion Electroacupuncture at the Ciliao (BL32) can ameliorate NBD in SCI rats, enhance rectal myoelectric activity, alleviate the intestinal inflammatory response, and upregulate TGR5 expression. The mechanism of electroacupuncture in improving NBD is closely associated with the upregulation of TGR5 expression. |
