| 基于李东垣阴火理论，本文探讨儿童组织细胞坏死性淋巴结炎的病机与治法。本病以脾胃虚弱、元气不足为发病之本；脾胃既虚，气机升降失常，郁而化火，阴火内生为其核心病机；阴火循经上扰、郁滞少阳枢机，致气机壅滞、痰瘀互结，发为淋巴结肿痛等临床诸症；正虚邪伏少阳，则为病程迁延、反复发作之由。结合“小儿脾常不足”的生理特点，确立补中升阳以固本、疏利少阳以透邪、清泻阴火以治标的治法。附验案一则，以期为儿童组织细胞坏死性淋巴结炎的中医治疗提供新的思路。 |

| Treatment of pediatric histiocytic necrotizing lymphadenitis based on LI Dongyuan's yin fire theory |

| Based on LI Dongyuan's yin fire theory, this article explores the pathogenesis and treatment methods of pediatric histiocytic necrotizing lymphadenitis(HNL). The root cause of the disease lies in spleen-stomach deficiency and Yuan-primordial qi insufficiency. When the spleen and stomach are deficient, it disrupts the normal ascending and descending movement of qi,resulting in qi stagnation and the subsequent generation of internal yin fire, which constitutes the core pathogenesis. The yin fire ascends along the meridians and stagnates at the Shaoyang pivot, resulting in qi stagnation along with intertwined phlegm and blood stasis. Clinically, this manifests as symptoms such as swollen and painful lymph nodes. The combination of deficient healthy qi and latent pathogenic qi in the Shaoyang meridian accounts for the prolonged and recurrent course of the disease. In light of the physiological characteristic that "children are often constitutionally deficient in the spleen," this article proposes treatment methods of tonifying the middle jiao and raising yang to consolidate the foundation, dredging and regulating the Shaoyang meridian to expel pathogenic factors, and clearing and draining yin fire to address the symptoms. A verified medical record is attached in the hope of providing new insights for Chinese medicine treatment of pediatric HNL. |
