| 变应性鼻炎（AR）病程迁延，常规治疗效果常不理想，究其原因与“郁-瘀-闭”的递进式病机演变密切相关。本文基于“玄府-肺络-三焦”轴，系统阐释该病的辨证施治思路。在此轴上，玄府郁闭为发病的起始环节，引发气机不畅与津液输布障碍；进而肺络瘀阻为核心病机，表现为痰瘀互结、风邪伏藏；终致三焦壅滞，出现气化功能障碍与水湿内停。三者互为因果，构成“郁-瘀-闭”恶性循环。治疗当以“开通玄府、宣通肺络、通调三焦”为总则，恢复气液宣通、络脉和利、三焦决渎之能，以期为AR的临床诊治提供新的理论框架与临床路径。 |

| Allergic rhinitis (AR) is characterized by a protracted clinical course and limited efficacy of conventional therapies. This is closely associated with the progressive pathological evolution involving "stagnation, stasis, and obstruction". Grounded in the "mysterious mansion-lung collaterals-sanjiao" theoretical framework, this study systematically expounds the principles of pattern identification and management for this condition. The pathogenic cascade begins with the occlusion of the sweat pores, leading to dysfunction of qi movement and disruption of body fluid metabolism. The core pathogenesis involves stagnation of the lung collaterals, manifesting as intermingled phlegm and blood stasis with latent wind pathogens. Disease progression further results in congestion of the sanjiao, impairing qi transformation and internal retention of water-dampness. These three components interact causally, forming a vicious cycle of "depression-stasis-blockage." The therapeutic strategy should therefore adhere to the principle of "unblocking the mysterious mansion, dredging the lung collaterals, and regulating the sanjiao," aiming to restore the normal diffusion of qi and fluids, re-establish harmony within the collateral system, and reinstate the drainage function of the sanjiao. This article offers a novel theoretical framework and practical pathway for the clinical management of AR. |
