| 外阴瘙痒是妇科疾病常见症状之一，归属于中医学“阴痒”范畴。本文基于“玄府-气液-神机”理论，系统探讨其内涵、中医病机与诊治思路，认为该病以玄府郁闭、气液不通、神机不遂为核心病机，主张以开玄府、通气液、安神机为治疗原则。临证分为三型论治：玄府初郁证治当疏郁开玄、濡润通利；玄府壅滞、湿热瘀结证治当开壅决滞、分化湿热；玄府失濡、神机失调证治当宁心安神、养阴通络。治疗时宜随证化裁。附验案一则，以期为外阴瘙痒的临床诊治提供新思路。 |

| Clinical approach to differentiating and treating pruritus vulvar from the perspective of the "mysterious mansion-qi and body fluids-vital spirit" theory |

| Pruritus vulvar (PV) is one of the common gynecological symptoms and is classified as "pubic itching" in TCM. Based on the "mysterious mansion-qi and body fluids-vital spirit" theory, this paper systematically explores its connotation, TCM pathogenesis, and approaches to diagnosis and treatment. The core pathogenesis of this disease is considered to involve obstruction of the mysterious mansion, impaired circulation of qi and body fluids, and dysfunction of vital spirit. Accordingly, the therapeutic principles emphasize unblocking the mysterious mansion, promoting the circulation of qi and body fluids, and regulating the vital spirit. Clinically, the treatment can be differentiated into three patterns: for the pattern of initial stagnation of mysterious mansion, the treatment focuses on removing stagnation, unblocking the mysterious mansion, and moistening to promote circulation; for the pattern of mysterious mansion obstruction with damp heat and blood stasis, the treatment aims to remove obstruction and eliminate stagnation, and resolve damp heat; for the pattern of insufficient nourishment of mysterious mansion with disordered vital spirit the treatment emphasizes calming the mind, nourishing yin, and unblocking the collaterals. The medication should be modified flexibly according to specific patterns. A verified medical record is presented, aiming to provide new ideas for the clinical diagnosis and treatment of PV. |
