| 摘要: |
| 筋膜-激痛点理论与中医“通则不痛”原则在软组织疼痛康复领域具有相似性。筋膜作为人体力学传递与疼痛传导的关键结构,其异常张力与激痛点形成密切相关,而中医“通则不痛”原则通过调节气血运行与组织微环境,为现代疼痛机制提供了传统理论支撑。本文整合中医理论与现代疼痛病理生理学研究,从病理关联与分子机制双重维度阐释软组织疼痛“通则不痛、痛则不通”的现代医学本质。筋膜炎症可引发筋膜纤维化与粘连、激痛点形成,并通过诱发能量危机与炎症微环境改变,进而导致生物力学、代谢及神经传导功能异常等问题,共同形成“气滞血瘀、经络不通”的病因病机,并提出“通则不痛”的实现是以松筋解结-功能重塑-整体改善为康复路径。研究构建“传统理论-现代机制-临床实践”的整合理论框架,为软组织疼痛康复提供更具科学性与实操性的理论支撑。 |

| Abstract: |
| The fascia-trigger point theory and TCM principle of "no obstruction, no pain" share similarities in the field of soft tissue pain rehabilitation. As a key structure for mechanical transmission and pain conduction in the human body, abnormal tension in the fascia is closely related to the formation of trigger points. Meanwhile, the TCM principle of "no obstruction, no pain" provides traditional theoretical support for modern pain mechanisms by regulating qi and blood circulation and tissue microenvironment. This paper integrates TCM theory with contemporary pathophysiological research on pain to elucidate the modern medical essence of soft tissue pain from the perspective of "no obstruction, no pain; pain indicates obstruction", examining it from the dual dimensions of pathological correlation and molecular mechanisms. Fascial inflammation can lead to fibrosis and adhesions of the fascia, as well as the formation of trigger points. By inducing the energy crisis and alterations in the inflammatory microenvironment, it subsequently causes abnormalities in biomechanical, metabolic, and neural conduction functions. These factors collectively constitute the etiology and pathogenesis of "blood stasis due to qi stagnation, and meridian blockage." It proposes that achieving the state of "no obstruction, no pain" follows a rehabilitation pathway of fascial relaxation and spasm relief functional remodeling-overall improvement. This study constructs an integrated theoretical framework of "traditional theory-modern mechanisms-clinical practice," aiming to provide more scientific and practical theoretical support for soft tissue pain rehabilitation. |
