| 动脉粥样硬化（AS）是引发冠心病、心肌梗死等心血管疾病的关键病理基础，线粒体形态和功能损伤是AS发生发展过程中的重要病理机制，同时也是激活巨噬细胞焦亡信号通路、触发免疫炎症反应的关键环节之一。本文基于“脾不散精”理论，从中西医不同角度解析AS的病理机制，认为线粒体损伤导致的功能障碍与“脾不散精”所致精微不布、痰浊内停高度相似；而巨噬细胞焦亡所驱动的免疫炎症反应与痰瘀互结一致。故基于AS“脾不散精”的病机理论，结合线粒体损伤与巨噬细胞焦亡的现代医学认识，阐释“助脾散精”的基本治则，以期赋予中医学“脾不散精”理论现代生物学内涵，并为临床防治AS提供新方向。 |

| Atherosclerosis (AS) is the primary pathological foundation for cardiovascular diseases such as coronary heart disease and myocardial infarction. Mitochondrial damage, both morphological and functional, constitutes a key pathological mechanism in the onset and progression of AS, and is also one of the key links in activating macrophage pyroptosis signaling pathways and triggering immune-inflammatory responses. Based on the theory of "spleen failing to disperse essence," this paper elucidates the pathological mechanism of atherosclerosis from both Chinese and Western medicine perspectives. It proposes that mitochondrial dysfunction caused by various damages closely mirrors the failure to distribute nutrients and the internal accumulation of turbid phlegm due to "spleen failing to disperse essence," while the immune-inflammatory responses driven by macrophage pyroptosis parallel the pattern of intertwined phlegm and blood stasis. Therefore, grounded in the pathogenic theory of "spleen failing to disperse essence" in AS, and integrated with modern medical insights into mitochondrial damage and macrophage pyroptosis, this paper elaborates the fundamental therapeutic principle of "assisting the spleen in dispersing essence." This aims to endow the Chinese medicine theory of "spleen failing to disperse essence" with modern biological connotations and to provide new directions for clinical prevention and treatment of AS. |
