“腰六针”电针法联合隔姜灸治疗寒湿型腰椎间盘突出症的临床研究 |

摘要: |
目的 探讨“腰六针”电针联合隔姜灸治疗寒湿型腰椎间盘突出症(lumbar disc herniation,LDH)的临床效果。方法 选取2018年7月至2020年7月本院诊治的寒湿型LDH患者182例,随机分为研究组、对照组(各91例),研究中途研究组脱落2例、对照组脱落3例。在常规治疗基础上,对照组行隔姜灸治疗,研究组行“腰六针”电针联合隔姜灸治疗,均治疗4周。观察比较两组的临床疗效、疼痛情况[视觉模拟评分(visual analog scale,VAS)]、症状及体征[日本骨科学会(Japan orthopaedic association,JOA)下腰痛评分]、腰椎功能[Oswestry功能障碍指数问卷表(Oswestry dysfunction index,ODI)]、腰椎活动度和安全性。结果 研究组总有效率(95.51%)高于对照组(79.55%),差异有统计学意义(P<0.05)。治疗1个疗程、2个疗程时,两组VAS评分均较之前下降(P<0.05)、JOA评分均较之前提高(P<0.05),且研究组VAS评分低于对照组(P<0.05)、JOA评分高于对照组(P<0.05)。治疗后,两组疼痛强度、提物、站立、生活自理、行走、睡眠、坐位评分及ODI总分均下降(P<0.05),且研究组上述评分均低于对照组(P<0.05)。治疗后,两组腰椎前屈、后伸、左旋、右旋活动度均增加(P<0.05),且研究组上述活动度均大于对照组(P<0.05)。治疗过程中研究组1例起针后轻微出血、对照组1例轻微皮肤瘙痒,未出现其他不良反应及严重应激表现。结论 “腰六针”电针联合隔姜灸治疗寒湿型LDH患者可明显缓解疼痛,改善症状、体征及腰椎功能,增加腰椎活动度,提高临床疗效,且安全可靠。 |

Abstract: |
Objective To explore the clinical effect of "lumbar six needles" electroacupuncture combined with ginger separated moxibustion in the treatment of cold-dampness type lumbar disc herniation (LDH). Methods 182 patients with cold-dampness type LDH treated in our hospital from July 2018 to July 2020 were randomly divided into study group and control group, with 91 cases in each group. During the study, 2 cases and 3 cases fell off in the study group and control group. On the basis of routine treatment, the control group was treated with ginger separated moxibustion, and the study group was treated with "lumbar six needles" electroacupuncture combined with ginger separated moxibustion. All patients were treated for 2 courses. The clinical efficacy, pain[visual analog scale (VAS)], symptoms and signs[Japan Orthopaedic Association (JOA) low back pain score], lumbar function[Oswestry dysfunction index (ODI)], lumbar mobility and safety were observed and compared between the two groups. Results The total effective rate of the study group (95.51%) was higher than that of the control group (79.55%), and the difference was statistically significant (P<0.05). After one course of treatment and two courses of treatment, the VAS score of the two groups decreased (P<0.05) and JOA score increased (P<0.05), and the VAS score of the study group was lower than that of the control group (P<0.05), and JOA score was higher than that of the control group (P<0.05). After treatment, the scores of pain intensity, extraction, standing, self-care, walking, sleep, sitting position and ODI total score in the two groups decreased (P<0.05), and the above scores in the study group were lower than those in the control group (P<0.05). After treatment, the lumbar flexion, extension, left rotation and right rotation activities in the two groups increased (P<0.05), and the above activities in the study group were greater than those in the control group (P<0.05). During the treatment, one case in the study group had slight bleeding after acupuncture and one case in the control group had slight skin pruritus. There were no other adverse reactions and severe stress. Conclusion "Lumbar six needles" electroacupuncture combined with ginger separated moxibustion in the treatment of cold-dampness type LDH patients can significantly relieve pain, improve symptoms, signs and lumbar function, increase lumbar mobility and improve clinical efficacy, which is safe and reliable. |
