从“脾为之卫”理论探讨肠道菌群在代谢性炎症中的作用 |

摘要: |
代谢性炎症作为一种慢性低度炎症状态,可能破坏人体内环境稳态与能量代谢平衡,进而引发代谢紊乱。肠道菌群在宿主能量代谢调控和免疫应答过程中发挥着重要作用,而调节肠道菌群可有效缓解代谢性炎症的发展。中医学理论认为脾为后天之本,并以此衍生了“脾为之卫”理论。“脾为之卫”理论深刻体现了脾通过运化水谷精微调控人体能量代谢,以及抵御外邪参与免疫调节的双重功能。这一理论与现代医学中肠道菌群的调控机制存在显著的关联性。当脾气亏虚导致脾卫功能失职时,机体的代谢调控与免疫平衡遭到破坏,促使炎症因子异常释放,最终诱发代谢性炎症。这种炎症过程具有缓慢持续的特征,长期发展可导致糖尿病等多种代谢性疾病的发生。中医药在代谢性炎症治疗中强调标本兼治的治则思路,以健脾益气固其本、化浊祛痰治其标。若因“脾为之卫”功能长期失职致使痰浊、瘀血等病理产物蓄积,则需采用滋阴解毒的治疗方法。深入探讨“脾为之卫”理论与肠道菌群在代谢性炎症发生发展中的相互作用机制,将为临床治疗提供重要的中医理论指导和实践智慧。 |

Abstract: |
Metabolic inflammation, a chronic low-grade inflammatory state, may disrupt internal homeostasis and energy metabolism balance, thereby triggering metabolic disorders. The gut microbiota plays a critical role in regulating host energy metabolism and immune responses, and modulation of the gut microbiota can effectively mitigate the progression of metabolic inflammation. According to Chinese medicine(CM) theory, the spleen is regarded as the foundation of acquired constitution, from which the theory of "the spleen acting as the guard of the body" is derived. This theory profoundly reflects the spleen’s dual functions: regulating energy metabolism through the transformation and transportation of water, food, and fluids, and defending against external pathogens by participating in immune regulation. This theory exhibits a significant correlation with the regulatory mechanisms of gut microbiota in modern medicine. Spleen qi deficiency leads to an impaired defensive function of the spleen,disrupting metabolic regulation and immune balance in the body. This results in the abnormal release of inflammatory factors and ultimately triggers metabolic inflammation. This inflammatory process is slow and persistent, and its long-term progression may contribute to the development of various metabolic diseases, such as diabetes mellitus. In treating metabolic inflammation, CM emphasizes the principle of addressing both the root cause and symptoms, focusing on strengthening the spleen and tonifying qi to strengthen the body, while transforming turbidity and resolving phlegm to alleviate symptoms. If the spleen’s guarding function remains compromised over an extended period, pathological products such as phlegm turbidity and blood stasis can accumulate,necessitating treatment strategies that nourish yin and remove toxins. Further in-depth exploration of the interaction mechanisms between the theory of "the spleen acting as the guard of the body" and the gut microbiota in the development and progression of metabolic inflammation will provide important theoretical guidance and practical insights from CM for clinical treatment. |
