| 摘要: |
| 总结郑亮教授从风邪动络理论辨治腹泻型肠易激综合征（IBS-D）的临证经验。郑亮教授认为，风邪动络、肠络失和为IBS-D的核心病机。初期以风湿相搏、气机逆乱、肠络拘急为主；中期风邪渐伏于络，与瘀血相结，兼见肝脾失调；后期则脾虚络弱、络虚不固，呈正虚邪恋之势。临证实行分期论治，初期重在疏风胜湿、调肝安络、缓急止痛，方选痛泻要方合藿香正气散化裁；中期重在祛风安络、理气活血、疏肝健脾，方选加味抑肝散化裁；后期则以健脾益气、固表御风、养络防复为要，方选参苓白术散合玉屏风散化裁。诸方随证加减，灵活化裁，临床疗效显著。附验案一则，以资佐证。 |

| ZHENG Liang's experience in differentiating and treating diarrhea-predominant irritable bowel syndrome from the theory of pathogenic wind stirring the collaterals |

| This paper summarizes Professor ZHENG Liang's clinical experience in differentiating and treating diarrhea-predominant irritable bowel syndrome (IBS-D) based on the theory of pathogenic wind stirring the collaterals. Professor ZHENG holds that pathogenic wind stirring the collaterals and disharmony of the intestinal collaterals constitute the core pathogenesis of IBS-D. In the early stage, wind-dampness contention, disordered qi movement, and spasm of the intestinal collaterals are predominant. In the middle stage, pathogenic wind gradually lurks in the collaterals and binds with static blood, accompanied by disharmony between the liver and spleen. In the late stage, the pattern is characterized by spleen deficiency and collateral weakness, with collateral deficiency and insecurity, presenting a state of healthy qi deficiency with lingering pathogen. In clinical practice, stage-based treatment is implemented. In the early stage, the focus is on dispersing wind and eliminating dampness, regulating the liver and calming the collaterals, and relieving spasm and alleviating pain; the selected formula is a modified combination of Tongxie Yaofang and Huoxiang Zhengqi Powder. In the middle stage, the focus is on eliminating wind and calming the collaterals, regulating qi and circulating blood, and soothing the liver and strengthening the spleen; the selected formula is modified Jiawei Yigan Powder. In the late stage, the key is to strengthen the spleen and supplement qi, secure the exterior and resist wind, and nourish the collaterals to prevent recurrence; the selected formula is a modified combination of Shenling Baizhu Powder and Yupingfeng Powder. All formulas are modified according to the differentiated patterns and flexibly adjusted, with remarkable clinical efficacy. A verified medical record is appended for corroboration. |
