| 摘要: |
| 子宫肌瘤为育龄期女性高发的良性生殖肿瘤。文章基于《金匮要略》干血理论“虚瘀互结、愈热愈干”的病理特征，结合月经周期热变规律，提出本病“以虚为本、以瘀为标、以热为变、以干血为果”的病机本质。确立“补虚化瘀、清热防变、序贯干预”的核心治法：经前期疏肝调气清郁热以断干血之基，行经期破血消癥逐瘀热以化干血之实，经后期攻补兼施消虚热以杜干血之复。附验案一则加以阐述，以期为子宫肌瘤的临证治疗提供新思路。 |

| Abstract: |
| Uterine fibroids (UF) are common benign tumors of the female reproductive system in women of reproductive age. Based on the pathological characteristics of intermingled deficiency and stasis and aggravation of dryness with increasing heat in the dry blood theory of Jin Gui Yao Lve (Essentials from the Golden Cabinet), together with the pattern of heat changes during the menstrual cycle, this article proposes that the pathogenesis of UF is characterized by deficiency as the root, stasis as the manifestation, heat as the pathological transmission, and dry blood as the consequence. Accordingly, this article establishes a core treatment approach centered on tonifying deficiency and transforming stasis, clearing heat and preventing transmission, and implementing sequential intervention: during the premenstrual phase, treatment focuses on soothing the liver, regulating qi, and clearing stagnant heat to eliminate the root of dry blood formation; during the menstrual phase, treatment focuses on breaking up blood, resolving abdominal masses, and dispelling stasis-heat to resolve dry blood accumulation; during the postmenstrual phase, treatment focuses on reinforcing healthy qi and removing pathogenic factors simultaneously, clearing deficiency heat, and preventing the recurrence of dry blood. A verified medical record is presented to elaborate on this approach, so as to provide a new approach to the clinical treatment of UF. |
