| 色素失禁症（IP）是一种罕见的X连锁显性遗传病，以特征性皮肤损害为主要表现，常伴发中枢神经系统异常，癫痫是其常见且严重的并发症。本文基于“虚气留滞”理论，结合IP的现代医学病理机制，探讨该病的中医病机，发现其以先天元气亏虚、肾精不足、脾肾两虚为本，以痰浊、瘀血、毒邪留滞脑络为标，本虚与标实之间相互影响，形成“因虚致滞、因滞更虚”的恶性循环，致癫痫反复发作。其病机演变为：初期胎禀脆弱，髓海空虚，肾精不充，脑络失养；中期脾肾阳虚，虚气不固，阳气亏乏，清阳不升，痰浊瘀血渐次内生；终期虚气急滞，毒损脑络，痰瘀毒邪互结，闭窍动风，痫证乃作。本文旨在为中西医结合治疗IP伴发癫痫提供理论参考。 |

| Incontinentia pigmenti (IP) is a rare X-linked dominant genetic disorder characterized predominantly by characteristic skin lesions, frequently accompanied by central nervous system abnormalities, with epilepsy being a common and serious complication. Based on the theory of "qi deficiency with stagnation" and integrating the modern pathological mechanisms of IP, this paper explores the TCM pathogenesis of the disease. It reveals that the root cause lies in congenital deficiency of Yuan-primordial qi, insufficiency of kidney essence, and dual deficiency of the spleen and kidney, while the manifestations are stagnation of phlegm-turbidity, stagnant blood, and toxic pathogens lingering in the brain collaterals. The interaction between root deficiency and manifestation excess forms a vicious cycle of "deficiency giving rise to stagnation, and stagnation exacerbating deficiency," leading to recurrent epileptic seizures. The evolution of pathogenesis can be delineated as follows: in the early stage, fetal constitution is fragile, the marrow sea is empty, kidney essence is insufficient, and the brain collaterals lose nourishment; in the middle stage, spleen-kidney yang deficiency weakens the defensive qi, yang qi is depleted, clear yang fails to ascend, and phlegm-turbidity and stagnant blood gradually generate internally; in the late stage, deficient qi becomes severely stagnated, toxic pathogens damage the brain collaterals, and phlegm, stagnant blood, and toxins intertwine, blocking the orifices and stirring wind, thus provoking seizure episodes. This paper aims to provide a theoretical reference for the integrated Chinese and Western medicine treatment of IP complicated with epilepsy. |
