| 目的 观察疏肝消瘿方治疗肝郁脾虚型桥本甲状腺炎(HT)伴焦虑抑郁状态患者的短期临床疗效与安全性。方法 选取2024年1月至2025年4月汕头市中医医院内分泌科门诊收治的60例符合纳入标准的肝郁脾虚型HT伴焦虑抑郁状态患者,采用随机数字表法将HT患者分为对照组与治疗组,每组30例。对照组予以健康教育、饮食作息指导等基础治疗,治疗组在对照组基础上加用疏肝消瘿方口服治疗,疗程4周。对比两组患者治疗前后中医证候积分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、血清中甲状腺过氧化物酶抗体(TPOAb)和甲状腺球蛋白抗体(TgAb)水平,以及记录不良反应,评估用药安全性。结果 治疗后,治疗组患者中医证候总有效率(86.67%)高于对照组(0%)(P<0.01)。治疗后,治疗组患者中医证候积分、SAS及SDS评分、TPOAb及TgAb水平均较治疗前降低(P<0.05),且治疗组均显著低于对照组(P<0.01)。两组患者治疗期间均未出现明显不良反应,用药安全性良好。结论 疏肝消瘿方短期干预可改善肝郁脾虚型HT伴焦虑抑郁状态患者的中医临床证候及焦虑抑郁状态,对甲状腺抗体水平也有一定调节作用,且临床用药安全。 |

| Objective To observe the short-term clinical efficacy and safety of Shugan Xiaoying Formula (SGXYF) in the treatment of patients with Hashimoto thyroiditis (HT) complicated with anxiety and depression of liver qi stagnation and spleen deficiency pattern. Methods Sixty patients with HT complicated with anxiety and depression conforming to the liver qi stagnation and spleen deficiency pattern, who were treated at the Outpatient Department of Endocrinology, Shantou Hospital of Traditional Chinese Medicine, from January 2024 to April 2025, were enrolled and randomly divided into control group and treatment group using a random number table, with 30 cases in each group. The control group received basic treatment including health education as well as diet and daily routine guidance, while the treatment group received additional oral administration of SGXYF on top of the basic treatment, with a treatment course of 4 weeks. Chinese medicine pattern scores, Self-Rating Anxiety Scale (SAS) scores, Self-Rating Depression Scale (SDS) scores, serum thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TgAb) levels were compared between the two groups before and after treatment. Adverse reactions were recorded to evaluate medication safety. Results After treatment, the total effective rate of TCM patterns in the treatment group (86.67%) was significantly higher than that in the control group (0%) (P<0.01). After treatment, TCM pattern scores, SAS scores, SDS scores, and TPOAb and TgAb levels in the treatment group all decreased compared with before treatment (P<0.05), and were significantly lower than those in the control group (P<0.01). No obvious adverse reactions occurred in either group during the treatment period, indicating favorable safety. Conclusion Short-term intervention with SGXYF improves TCM clinical symptoms and anxiety-depression states in HT patients with liver qi stagnation and spleen deficiency pattern, and also exerts certain regulatory effects on thyroid antibody levels, with good clinical safety. Further in-depth studies are warranted. |
