| 摘要: |
| 多囊卵巢综合征（PCOS）伴发的高雄激素血症是复杂的女性内分泌代谢性疾病。现代医学治疗多以抗雄激素及调节代谢为主。本文立足《伤寒论》三阴辨证理论，综述该病的中医病机演变与经方论治规律，认为该病实质为三阴脏腑虚寒网络交织：太阴脾虚湿盛，与糖脂代谢紊乱及胰岛素抵抗高度契合；少阴肾精亏虚，乃下丘脑-垂体-卵巢（HPO）轴失调之本源；厥阴寒热错杂，则反映了神经内分泌与免疫应激的网络级联反应。基于此多维病机，本文综述了理中汤、肾气丸、乌梅丸等经方的靶向干预机制，旨在跨越单一靶点治疗的局限，为中医药系统治疗PCOS高雄激素血症提供新的临床思路。 |

| Polycystic ovary syndrome (PCOS)-associated hyperandrogenism is a complex female endocrine and metabolic disorder. Current modern medical treatments primarily focus on anti-androgen therapies and metabolic regulation. Based on the three-yin pattern differentiation theory from the Shang Han Lun (Treatise on Cold Damage), this paper systematically reviews the TCM pathogenesis evolution and classical formula treatment rules of this disease. It proposes that the pathological essence of this disease lies in the interwoven network of deficiency and cold across the three yin zang organs: Taiyin spleen deficiency with dampness exuberance, which closely corresponds to glycolipid metabolism disorders and insulin resistance; Shaoyin kidney essence deficiency, which serves as the root of hypothalamic-pituitary-ovarian (HPO) axis dysregulation; and Jueyin cold-heat complexity, which reflects the network cascade of neuroendocrine and immune stress responses. Based on this multidimensional pathogenesis, this paper reviews the targeted intervention mechanisms of classical formulas such as Lizhong Pill, Shenqi Pill, and Wumei Pill, aiming to transcend the limitations of single-target therapy and provide new clinical perspectives for the systematic TCM treatment of PCOS-associated hyperandrogenemia. |
