| 摘要: |
| 肺癌合并阻塞性肺炎是由于肿瘤压迫、分泌物滞留及机体免疫力下降等因素引起的远端肺实质感染，抗肿瘤药物与敏感抗菌药物双重使用作为当前主要的治疗手段，仍会出现耐药或反复感染。中医药协同西医疗法在控制病情进展与改善患者长期预后方面发挥不可替代的协同作用。刘丽坤教授强调，晚期肺癌合并阻塞性肺炎以肺元亏虚、宣肃功能废弛为发病之根本。中州衰惫，土不养金；肝气逆乱，气机升降失序，皆可导致肺治节无权。其中痰瘀浊邪停聚为病势加重的关键，虚实夹杂、积久酿毒为本病之标。刘丽坤教授主张以“枢机-气机-络机”通调法，肺、脾、肝同治，运用柴芍六君子汤合补肺汤化裁补虚泻实，治宜培土生金、清通开宣、涤痰化瘀，截断病势，恢复肺之宣肃治节的生理功能，临床力专效宏。附验案一则，以飨同道。 |

| Abstract: |
| Lung cancer complicated with obstructive pneumonia is a distal lung parenchymal infection caused by tumor compression, secretion retention, and decreased immunity. Although the combined administration of antineoplastic agents and sensitive antibacterial drugs is the current mainstay of treatment, drug resistance or recurrent infections still occur. Integrated Chinese medicine (TCM) and Western medicine plays an irreplaceable synergistic role in controlling disease progression and improving long-term outcomes. Professor LIU Likun emphasizes that for advanced lung cancer complicated with obstructive pneumonia, the root lies in deficiency and exhaustion of lung primordial qi and the failure of the lung's diffusing and descending functions. Debility of the middle energizer (spleen-stomach) results in failure of earth (spleen) to nourish metal (lung); liver qi reversal and chaos lead to disordered ascent and descent of qi movement. Both can cause the lung to lose its management and regulation, allowing phlegm, blood stasis, and turbid pathogens to accumulate—the key to disease aggravation. Intermingled deficiency and excess with prolonged accumulation brewing toxin constitute the manifestations of the disease. Professor LIU advocates a "pivot-qi movement-collateral" regulation method to simultaneously treat the lung, spleen, and liver, applying modified Chaishao Liujunzi Decoction combined with Bufei Decoction to tonify deficiency and purge excess. The treatment principles are tonifying the spleen and benefiting the lung, clearing, unblocking, and diffusing the lung qi, and cleansing phlegm and transforming stasis, thereby truncating disease progression and restoring the lung's physiological functions of diffusion, descent, management, and regulation, achieving substantial clinical efficacy. A clinical case is appended for the reference of fellow practitioners. |
