| 梅尼埃病是一种以内耳膜迷路积水为特征的难治性耳源性眩晕性疾病，临床反复发作、迁延难愈。西医常规治疗疗效有限，且易复发。伍大华教授结合多年临床经验提出，肾精亏虚为本病的发病之本、精虚生风为发作的内在机制、清窍闭阻为病机的转化枢纽，三者形成“精虚-风动-窍闭”的病机链条。据此提出“以补肾填精为本、息风通窍为用”的一体化辨治思路，强调填精以制内风、补虚以助窍通，临证注重顾护脾胃以保障气血生化，使补而不滞、通而不伤。本文系统总结伍大华教授诊治梅尼埃病的病机认识、治则方药、用药规律及临证验案，为中医药临床诊疗梅尼埃病提供一定的参考价值。 |

| Ménière's disease (MD) is a refractory otogenic vertiginous disorder characterized by endolymphatic hydrops of the inner ear, with recurrent episodes and a prolonged, difficult-to-cure clinical course. Conventional Western medical treatment has limited efficacy and a high relapse rate. Professor WU Dahua, based on years of clinical experience, proposes that kidney essence deficiency is the root cause of MD, that essence deficiency generating wind is the intrinsic mechanism of disease onset, and that blockage of the clear orifices serves as the pivotal link in the pathological transformation. These three elements form a pathogenic chain of "essence deficiency-wind stirring-orifice blockage." Accordingly, she has established an integrated differentiating and treating approach centered on "tonifying the kidney and supplementing essence as the foundation, and extinguishing wind and opening the orifices as the application." He emphasizes that supplementing essence can subdue endogenous wind, while supplementing deficiency can facilitate orifice opening. In clinical practice, he pays special attention to nurturing and protecting the spleen and stomach to ensure the generation of qi and blood, thereby achieving tonification without stagnation and unblocking without impairment. This study systematically summarizes Professor WU's understanding of the pathogenesis, treatment principles, formulas and medicinals, medication regularity, and verified clinical medical records in the diagnosis and treatment of Ménière's disease, aiming to provide a reference for clinically diagnosing and treating this disease with Chinese medicine. |
