| 动脉粥样硬化(AS)是心脑血管疾病的重要病理基础,斑块稳定性直接决定疾病的预后,胞葬作用异常是造成斑块失稳的微观机制。本病在中医学归属于“脉痹”“痰浊”“瘀阻”范畴,病机为水郁内停、痰瘀互结、脉道壅塞,与胞葬功能失常引发病理产物蓄积的病理过程契合。本文以“水郁折之”理论为切入点,梳理胞葬作用紊乱影响AS斑块稳定性的现代机制,阐释“水郁折之”理论与本病病机的内在关联,进而确立疏郁化浊、通脉稳斑的中医治则,拟定对应方药并解析配伍思路、临证加减方案。同时,从调控胞葬通路、抑制炎症、调节脂质代谢、修复斑块结构等方面阐明方药作用机理。从“水郁折之”角度论治AS可实现宏观辨证与微观机制相结合,多靶点调控胞葬功能、稳定AS斑块,为中医药防治本病提供理论依据与临床参考。 |

| TCM differentiation and treatment strategies for efferocytosis and atherosclerotic plaque stability from the perspective of “dispersing water stagnation” |

| Atherosclerosis (AS) is a critical pathological basis for cardiovascular and cerebrovascular diseases. Plaque stability directly determines the prognosis, and efferocytosis dysfunction is the microscopic mechanism underlying plaque destabilization. In Chinese medicine (TCM), AS falls into the categories of vessel impediment, phlegm turbidity, and stasis obstruction. Its pathogenesis involves internal water stagnation, intertwined phlegm and blood stasis, and obstruction of the vessels, which corresponds to the pathological process consistent with the accumulation of pathological products resulting from efferocytosis dysfunction. This paper takes the theory of "dispersing water stagnation" as the entry point, reviews the modern mechanisms by which dysregulated efferocytosis affects AS plaque stability, and elucidates the intrinsic link between this theory and the disease pathogenesis. On this basis, it establishes the TCM treatment principles of dispersing stagnation and transforming turbidity, as well as unblocking the vessels and stabilizing plaques. Corresponding formulas are proposed, and their compatibility rationale and pattern-based modifications are analyzed. Furthermore, the mechanisms of action of these formulas are elucidated from multiple aspects, including regulation of efferocytosis pathways, inhibition of inflammation, modulation of lipid metabolism, and repair of plaque structure. Treating AS from the perspective of "dispersing water stagnation" integrates macroscopic pattern differentiation with microscopic mechanisms, enabling multi-target regulation of efferocytosis and AS plaque stabilization, thus providing a theoretical basis and clinical reference for the TCM prevention and treatment of this disease. |
