| 散风通窍滴丸联合鼻内针刺治疗中重度变应性鼻炎效果及对患者血清总IgE细胞免疫功能的影响 |

| 目的 观察散风通窍滴丸联合鼻内针刺对中重度变应性鼻炎(AR)患者的临床效果及对血清总免疫球蛋白E(IgE)、细胞免疫功能的影响。方法 纳入2022年1月至2024年1月南京中医药大学附属苏州市中医医院收治的AR患者104例为研究对象,随机分为对照组和观察组,各52例。对照组口服散风通窍滴丸,20丸/次,3次/d;观察组在此基础上联合鼻内针刺内迎香、鼻丘(均为双侧),1次/d,每周5次。两组均连续治疗2周。治疗后,比较两组患者鼻部症状总评分(TNSS)、鼻结膜炎生活质量问卷(RQLQ)评分、眼部症状总评分(TOSS),血清总IgE、嗜酸性粒细胞(EOS)、γ干扰素(IFN-γ)、白细胞介素(IL)-4、IL-10、IL-13水平,T淋巴细胞CD4+、CD8+、CD4+/CD8+水平,12、24周复发率,临床疗效及不良反应。结果 与治疗前比较,治疗后,两组TNSS、TOSS、RQLQ评分、血清总IgE、EOS、IL-4、IL-13、CD4+/CD8+水平均降低,IFN-γ、IL-10、CD4+、CD8+水平均升高(P<0.05);与对照组比较,观察组TNSS、TOSS、RQLQ评分及血清总IgE、EOS、IL-4、IL-13、CD4+/CD8+水平降低,IFN-γ、IL-10、CD4+、CD8+水平升高(P<0.05)。随访期间观察组失联2例,对照组失联1例,两组12周复发率差异无统计学意义(P>0.05)。观察组24周复发率(22.00%)低于对照组(49.02%)(P<0.05)。观察组总有效率(94.23%)高于对照组(80.77%)(P<0.05)。两组患者均未出现严重不良反应。结论 散风通窍滴丸联合鼻内针刺治疗中重度AR患者临床疗效优于散风通窍滴丸,其能在保证安全的基础上降低炎症水平、调节细胞免疫功能,从而纠正鼻炎相关症状,提高生活质量,改善预后。 |

| Objective To observe the clinical effects of Sanfeng Tongqiao Dropping Pills (SFTQDP) combined with intranasal acupuncture on patients with moderate to severe allergic rhinitis (AR) and the effects on serum total immunoglobulin E (IgE) and cellular immune function. Methods A total of 104 AR patients admitted to Suzhou Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from January 2022 to January 2024 were enrolled as study subjects and randomly divided into control group and observation group, with 52 cases in each group. The control group orally received SFTQDP, 20 pills per dose, three times a day. The observation group additionally received intranasal acupuncture at Neiyingxiang (EX-HN9) and Biqiu (both bilateral), once a day, five times per week, on the basis of the control group's treatment. Both groups were treated continuously for two weeks. After treatment, the two groups were compared in terms of total nasal symptom score (TNSS), rhinoconjunctivitis quality of life questionnaire (RQLQ) score, total ocular symptom score (TOSS), serum levels of total IgE, eosinophil count (EOS), interferon-γ (IFN-γ), interleukins (IL)-4, IL-10, and IL-13 levels, T-lymphocyte CD4+, CD8+, and CD4+/CD8+ levels, recurrence rates at 12 and 24 weeks, clinical efficacy, and adverse reactions. Results Compared with before treatment, both groups showed decreases in TNSS, TOSS, RQLQ scores, serum total IgE, EOS, IL-4, IL-13, and CD4+/CD8+ levels, and increases in IFN-γ, IL-10, CD4+, and CD8+ levels after treatment (P<0.05). Compared with the control group, the observation group showed lower TNSS, TOSS, RQLQ scores, serum total IgE, EOS, IL-4, IL-13, and CD4+/CD8+ levels, and higher IFN-γ, IL-10, CD4+, and CD8+ levels (P<0.05). During follow-up, 2 cases were lost to follow-up in the observation group and 1 case in the control group; there was no significant difference in the 12-week recurrence rate between the two groups (P>0.05). The 24-week recurrence rate in the observation group (22.00%) was lower than that in the control group (49.02%) (P<0.05). The total effective rate in the observation group (94.23%) was higher than that in the control group (80.77%) (P<0.05). No serious adverse reactions occurred in either group. Conclusion SFTQDP combined with intranasal acupuncture is superior to SFTQDP alone in treating patients with moderate-to-severe AR. It can reduce inflammation levels and regulate cellular immune function on the basis of ensuring safety, thereby alleviating rhinitis-related symptoms, improving quality of life, and enhancing prognosis. |
