| 老年主观性耳鸣病机复杂、病程缠绵，为临床常见的难治性疾病。陈国丰教授基于“寒气生浊”理论辨治本病，认为其以脾肾阳虚为本，寒邪内袭为标，从而构建分期论治体系，将其病机演变划分为三期：肇始期为寒邪犯肺，浊闭清窍；进展期为脾肾阳虚，寒浊内生；顽固期为浊化为毒，胶结难解。基于此提出“宣肺化浊，通络止鸣”“温阳化浊，培本固元”“化痰散瘀，驱散浊毒”三大治法，全程贯穿扶正祛邪之旨，临床疗效显著。附验案1则以佐证。 |

| Senile subjective tinnitus has a complex pathogenesis and a protracted clinical course, making it a common refractory disease in clinical practice. Professor CHEN Guofeng differentiates and treats this condition based on the theory of "cold qi generating turbidity". He proposes that the root cause lies in yang deficiency of the spleen and kidney, while the manifestation is internal invasion of cold pathogen. On this basis, he has established a stage-based treatment system, dividing the pathogenesis evolution into three phases: the initial phase, characterized by cold pathogen attacking the lung and turbidity obstructing the clear orifices; the progressive phase, marked by yang deficiency of the spleen and kidney with endogenous generation of cold turbidity;and the refractory phase, in which turbidity transforms into toxin, leading to intractable stagnation. Accordingly, he proposes three therapeutic principles: "dispersing the lung, resolving turbidity, and unblocking the collaterals to stop tinnitus"; "warming yang,resolving turbidity, and strengthening the root to consolidate Yuan-primordial qi"; and "transforming phlegm, dissipating stasis, and expelling turbid toxin." Throughout all stages, the approach consistently emphasizes reinforcing healthy qi to eliminate pathogenic factors, achieving notable clinical efficacy. One verified medical record is appended for illustration. |
