| 摘要: |
| 目的 探究脐周八穴针疗法对腹型肥胖者内脏脂肪的调节作用。方法 选取2022年10月至2025年8月四川省中西医结合医院收治的122例腹型肥胖患者作为研究对象。对照组采用生活方式指导，观察组采用生活方式指导联合脐周八穴针疗法，经倾向性评分匹配（PSM）后每组纳入48例。比较两组患者体围（腰围、臀围、腰臀比），肥胖指标[体质量、身体质量指数（BMI）、肥胖度]，血脂指标[总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白胆固醇（LDL-C）及高密度脂蛋白胆固醇（HDL-C）]，生活质量评分、暴食倾向评分、内脏脂肪指标变化及临床疗效。结果 与治疗前比较，治疗后两组患者腰围、体质量、BMI、臀围、腰臀比、肥胖度、TC、TG、LDL-C、腹主动脉周围脂肪组织（APAT）体积、胸主动脉周围脂肪组织（TPAT）体积、心外膜脂肪组织（EAT）体积、腹部内脏脂肪组织（VAT）体积、冠状动脉周围脂肪厚度、右心室游离壁脂肪厚度、体重相关生活质量量表简表（IWQOL-Lite）及食物渴求特质问卷（FCQ-T）评分均降低（P<0.05）;HDL-C水平升高（P<0.05）。与对照组比较，治疗组患者腰围、体质量、BMI、臀围、腰臀比、肥胖度、TC、TG、LDL-C、APAT体积、TPAT体积、EAT体积、VAT体积、冠状动脉周围脂肪厚度、右心室游离壁脂肪厚度、IWQOL-Lite及FCQ-T评分均降低（P<0.05）;HDL-C水平升高（P<0.05）。治疗后，观察组总有效率高于对照组（P<0.05）。结论 脐周八穴针刺疗法可能通过改善常规肥胖指标，减少腹腔和胸腔的内脏脂肪分布从而有效干预腹型肥胖，为针刺调节内脏脂肪提供循证医学证据。 |

| Effects of acupuncture at eight periumbilical acupoints on visceral fat in patients with abdominal obesity |

| Abstract: |
| Objective To investigate the regulatory effects of acupuncture at eight periumbilical acupoints on visceral fat in patients with abdominal obesity. Methods A total of 122 patients with abdominal obesity admitted to Sichuan Integrative Medicine Hospital from October 2022 to August 2025 were selected as the research subjects. The control group received lifestyle intervention alone, while the observation group received a combination therapy of lifestyle intervention and acupuncture at eight periumbilical acupoints. After propensity score matching(PSM), 48 cases remained in each group. Body circumference parameters(waist circumference, hip circumference, waist-hip ratio), obesity indicators(body mass, body mass index(BMI), degree of obesity), blood lipid indicators(total cholesterol(TC), triglycerides(TG), low-density lipoprotein cholesterol(LDL-C), and high-density lipoprotein cholesterol(HDL-C)), quality of life scores, binge eating tendency scores, changes of visceral fat indicators, and clinical efficacy were compared between the two groups of patients. Results Compared with pre-treatment levels, both groups showed decreases in waist circumference, body mass, BMI, hip circumference, waist-hip ratio, degree of obesity, TC, TG, LDL-C, volume of adipose tissue surrounding the abdominal aorta(APAT), volume of adipose tissue surrounding the thoracic aorta(TPAT), epicardial adipose tissue(EAT) volume, visceral adipose tissue(VAT) volume, pericoronary fat, right ventricular free wall fat thickness, and scores on the impact of weight on quality of life-lite(IWQOL-Lite) and the food craving questionnaire-trait(FCQ-T) after treatment(P<0.05), while HDL-C levels increased(P<0.05). Compared with the control group, the aforementioned parameters of the observation group were lower(P<0.05), while HDL-C levels were higher(P<0.05). After treatment, the total efficacy of observation group was higher than that of control group(P<0.05). Conclusion Acupuncture at eight periumbilical acupoints may be an effective intervention strategy for abdominal obesity by improving conventional obesity indicators and reducing visceral fat distributed in both the abdominal and thoracic cavities. This paper aims to provide evidence-based support for the efficacy of acupuncture in regulating visceral fat. |
