| “执中御下”法是国医大师杨春波教授基于“脾病则九窍不利”的大脾胃观、针对下焦湿热病提出的治疗大法，丰富了中医脾胃学说的临床内涵。湿热型良性前列腺增生症（BPH）为临床常见病、难治病，杨春波教授以脾胃为核心，系统论述了该分型之病因病机以中焦脾胃湿热为根，流注下焦、化生瘀痰为源；同时易夹阴亏、肝郁、肾虚之兼证，审证时当谨守四诊以辨之。治疗上需执脾胃以御下焦，重在清化中焦湿热，佐以消散下焦瘀痰；还需注意顾护阴液、疏肝解郁、平补肾元。附验案一则，以期为湿热型BPH的中医辨治提供参考经验。 |

| The method of "governing the middle jiao to regulate the lower jiao" is a therapeutic strategy proposed by Professor YANG Chunbo, a National TCM Master, based on the grand concept of "spleen disorders lead to dysfunction of the nine orifices." It targets damp heat diseases in the lower jiao and enriches the clinical connotation of the TCM spleen-stomach theory.Benign prostatic hyperplasia(BPH) of the damp heat pattern is a common and refractory clinical condition. Professor YANG, focusing on the spleen and stomach as the core, systematically elaborates that the etiology and pathogenesis of this pattern are rooted in spleen and stomach damp heat in the middle jiao, which flows downward to generate blood stasis and phlegm in the lower jiao.Meanwhile, it is often accompanied by concurrent patterns such as yin deficiency, liver qi stagnation, and kidney deficiency. Pattern differentiation should be conducted with the four examinations to precisely identify the condition. Treatment should adhere to governing the middle jiao(spleen and stomach) to regulate the lower jiao, emphasizing clearing and transforming damp heat in the middle jiao while assisting in dissipating blood stasis and phlegm in the lower jiao. Attention should also be paid to protecting yin fluids, soothing the liver to relieve depression, and gently tonifying the kidney. A verified medical record is provided, aiming to offer reference experience for the TCM pattern differentiation and treatment of BPH of the damp heat pattern. |
