| 总结王宏坤教授治疗膝骨关节炎（KOA）的临证经验。王宏坤教授提出，KOA的核心病机为“本痿标痹”，即以肝肾亏虚、气血不足为本，以风寒湿邪、瘀血痹阻为标。治疗上，以“扶痿祛痹、标本同治”为纲。内治方面，善用药对配伍，构建了调补肝肾脾胃、外祛风寒瘀血的立体方药体系；外治方面，秉承“筋骨并重”理念，运用理筋推拿手法松解关节周围经筋结聚，以疏通气血、滑利关节。内治与外治相结合，共奏“筋柔骨正、气血畅达”之效，临床疗效显著。附验案一则，为KOA的中医诊疗提供临床经验与借鉴。 |

| This paper summarizes the clinical experience of Professor WANG Hongkun in treating knee osteoarthritis (KOA). Professor WANG proposes that the core pathogenesis of KOA is "root-wei-flaccidity and manifestation-bi-impediment," which means that the root cause lies in liver and kidney deficiency along with qi and blood insufficiency, while the manifestations involve pathogenic factors such as wind, cold, dampness, and blood stasis obstructing the meridians. In terms of treatment, he adheres to the principle of "strengthening the wei-flaccidity condition and dispelling bi-impediment, treating both the root and manifestation simultaneously." For internal treatment, he excels in the use of medicinal pair combinations to establish a comprehensive medicine-formula system that regulates and nourishes the liver, kidney, spleen, and stomach while dispelling wind, cold, and blood stasis externally. In terms of external therapy, adhering to the principle of "equal emphasis on sinews and bones," he applies sinew-regulating manipulation to relax the sinew accumulations around the joints, thereby promoting qi and blood circulation and smoothing joint movement. The combination of internal and external treatments works synergistically to achieve "sinew flexibility, bone alignment, and unobstructed qi and blood circulation," yielding remarkable clinical results. A verified medical record is attached to provide clinical experience and reference for the TCM treatment of KOA. |
