| 摘要: |
| 卒中后肢体运动功能障碍是指脑卒中后以上运动神经元损伤为特征，以肢体运动不协调、肌张力异常、反射亢进等为常见表现的后遗症。本文从“髓海-小周天”系统理论探讨卒中后肢体运动功能障碍的病机演变及针灸治疗策略，认为“髓海-小周天”系统与“脑-脊髓-神经轴”的调控机制存在显著对应关系，推知本病的核心病机为髓海失充、小周天失畅，从而导致神机失用、气血失司、肢体痿废。治疗旨在充髓海、畅周天，提出了“三期三法”的针灸辨证治疗体系，具体方法包括透刺法、触骨针法、快针刺法、梅花针循经叩刺法、任督周天大艾灸法。通过“三期三法”的辨治，使髓海盈满，小周天循环畅通，则疾病转愈。 |

| Post-stroke limb motor dysfunction refers to a condition characterized by upper motor neuron damage following a stroke, sequelae characterized by common manifesting such as limb movement incoordination, abnormal muscle tone, and hyperreflexia. This paper explores the pathogenesis and acupuncture treatment strategies for post-stroke limb motor dysfunction based on the theory of the "sea of marrow-microcosmic orbit" system. It posits a significant correspondence between the regulatory mechanisms of the "sea of marrow-microcosmic orbit" system and the "brain-spinal cord-nerve axis". It is inferred that the core pathogenesis of this disease lies in the insufficiency of the sea of marrow and obstruction of microcosmic orbit circulation, leading to impaired mental function, dysregulation of qi and blood, as well as limb flaccidity and disuse. The treatment aims to replenish the sea of marrow and unblock the microcosmic orbit circulation. Accordingly, a "three-phase and three-method" acupuncture-moxibustion pattern differentiation and treatment system is proposed. Specific techniques include point-toward-point needling, bone-touch needling, rapid needling, plum-blossom needle tapping along meridians, and moxibustion along the Ren and Du meridians for the microcosmic orbit. Through this "three-phase and three-method" approach, replenishment of the sea of marrow and unimpeded circulation of the microcosmic orbit are achieved, thereby leading to the recovery from the disease. |
