| 摘要: |
| “风气通于肝”理论是指风气与肝主疏泄的功能相通，风气可濡养肝木，在病理变化过程中风气与肝属于同类，外风易引动内风，气同则合，内外相召。肉芽肿性乳腺炎的发生和复发与风气失常有着密切关系，肝郁化风、痰瘀暗生为发病之肇始，内外相召、暗瘀宿痰为发病之关键，正气虚馁、伏风稽留为复发之宿根。在治疗上应从“疏肝郁、息内风”“散外风、防相召”“豁痰浊、逐瘀血”“扶正气、搜伏风”“畅情志、和五味”5个方面入手，在缩小肿块、缩短病程和预防复发等方面具有良好的临床疗效。附验案一则，以飨同道。 |

| Exploring the treatment of granulomatous mastitis from the perspective of the "wind connects with the liver" theory |

| The theory "wind connects with the liver" posits that wind corresponds to the liver's function of governing the free flow of qi, thereby nourishing the liver-wood system. In pathological processes, wind and the liver belong to the same category; external wind tends to stir internal wind, and due to their shared nature, they combine and interact, resulting in mutual reinforcement between internal and external factors. The onset and recurrence of granulomatous mastitis are closely related to the dysfunction of wind. It is proposed that liver qi stagnation transforming into wind, accompanied by the insidious generation of phlegm and stasis, constitutes the initial trigger of the disease. The mutual induction of internal and external wind, which agitates phlegm and stasis, represents the key pathogenic mechanism. Deficient healthy qi and lingering latent wind serve as the deep-rooted cause of recurrence. In treatment, the approach should focus on five key aspects: "soothing the liver and regulating qi, and extinguishing internal wind", "dispersing external wind and preventing mutual induction", "resolving phlegm-turbidity and removing blood stasis", "supporting healthy qi and searching out latent wind", and "regulating emotions and harmonizing the five flavors". This comprehensive strategy demonstrates favorable clinical efficacy in reducing mass size, shortening disease duration, and preventing recurrence. A representative case example is appended for peer reference. |
