| 老年原发性干燥综合征常因症状隐匿、多病共存而易被漏诊，表现为口干、眼干及多系统损害。本文基于叶天士“甘守津还”理论，认为玄府以开为顺、以阖为逆，并提出其核心病机为脾肾虚衰、玄府闭密、气液失宣，以致津亏液涸、燥毒痰瘀互结，形成本虚标实、多病并存之候。治疗上以“甘守津还”为纲，立甘寒濡润、甘苦泄毒、甘辛通玄、甘淡运脾四法，旨在“开玄致津”，兼顾津液化生与输布，恢复气液宣通。 |

| Elderly patients with primary Sjögren’s syndrome are often prone to missed diagnosis due to their concealed symptoms and the coexistence of multiple diseases, presenting with dry mouth, dry eyes, and multi-system impairments. Based on YE Tianshi’s "Gan Shou Jin Huan"（sweet medicines nourish and restore body fluid） theory, this paper posits that the normal state of the mysterious mansion is openness, while closure is pathological. It proposes that the core pathogenesis involves spleen and kidney deficiency, closure of mysterious mansion, and impaired distribution of qi and body fluids, leading to body fluid depletion,the intermingling of dryness-toxin, phlegm, and blood stasis, and resulting in a condition characterized by both deficiency in the root and excess in the manifestations, along with the coexistence of multiple diseases. In terms of treatment, guided by the principle of "Gan Shou Jin Huan", four therapeutic methods are established: nourishing with sweet-cold and moistening medicines, removing toxins with sweet-bitter medicines, opening the mysterious mansion with sweet-pungent medicines, and regulating the spleen with sweet-bland medicines. The aim is to "open the mysterious mansion to restore body fluids", taking into account both the generation and distribution of body fluids, and restoring the normal flow of qi and body fluids. |
