| 摘要: |
| 目的 分析2017-2020年湖南省省级中医院次均住院费用内部构成及变化，为进一步优化次均住院费用结构、控制医疗费用膨胀提供参考，同时为促进中医院的发展，充分发挥中医药优势的相关政策提供实证支撑。方法 提取2017-2020年所有住院患者的病案首页数据，采用灰色关联分析法和结构变动度分析法进行评价。结果 2017-2020年湖南省省级中医院次均住院费用逐年增加，但增速放缓；各分项费用中关联度最大的是诊断费（1.000 0），其次是西药费（0.979 8）、综合医疗服务费（0.952 3），关联度最低的是血液和血制品类（0.868 2）；在次均住院费用中，诊断费、治疗费、西药费、中药费、耗材费所占比重在上升；4年间，次均住院费用结构变动度为13.57%，综合医疗服务费、耗材费、中药费是引起次均住院费用结构变动的主要因素，三者累计贡献率为60.13%。结论 湖南省省级中医院次均住院费用逐年增加，但增速放缓，需继续控制医疗费用总量；中医特色优势与医务人员劳务价值未充分体现，需协同推进医疗服务价格调整和付费方式改革。 |

| Analysis on the change of average hospitalization expenses of provincial traditional Chinese medicine hospitals in Hunan Province from 2017 to 2020 |

| Abstract: |
| Objective To analyze the internal composition and changes of the average hospitalization expenses of provincial traditional Chinese medicine hospitals in Hunan Province from 2017 to 2020, to further optimize the average hospitalization expenses structure and control the expansion of medical expenses, and to provide empirical support for the formulation of relevant policies to promote the development of traditional Chinese medicine hospitals and give full play to the advantages of traditional Chinese medicine. Methods The first page data of medical records of all inpatients from 2017 to 2020 were extracted and evaluated by grey correlation analysis and structural change analysis. Results From 2017 to 2020, the average hospitalization expenses of provincial traditional Chinese medicine hospitals in Hunan Province increased year by year, but the growth rate slowed down; among the itemized expenses, the diagnosis fee (1.000 0) had the greatest relevance, followed by western medicine fee (0.979 8) and comprehensive medical service fee (0.952 3), and the blood and blood products category (0.868 2) had the lowest correlation; in the average hospitalization expenses, the proportion of diagnosis expenses, treatment expenses, western medicine expenses, traditional Chinese medicine expenses and consumables expenses were increasing; in the past four years, the change degree of average hospitalization cost structure was 13.57%, comprehensive medical service fee, consumables fee and traditional Chinese medicine fee were the main factors causing the change of average hospitalization cost structure, and the cumulative contribution rate of the three was 60.13%. Conclusion The average hospitalization expenses of provincial traditional Chinese medicine hospitals in Hunan Province increase year by year, but the growth rate slows down, so it is necessary to continue to control the total amount of medical expenses; the characteristic advantages of traditional Chinese medicine and the labor value of medical personnel are not fully reflected, so it is necessary to promote the adjustment of medical service price and the reform of payment mode. |
