| 摘要: |
| 目的 探讨炙甘草汤联合常规西医治疗慢性心衰合并抑郁症的临床疗效。方法 选择2017年3月至2019年3月在本院接受治疗的慢性心力衰竭合并抑郁症患者89例进行研究。根据患者接受治疗的不同，将其分为2组，其中对照组45例接受常规西医治疗，观察组44例在常规西医治疗基础上加以炙甘草汤治疗。对比两组患者临床疗效、中医症状体征、生活质量、抑郁情况、生化指标及安全性指标。结果 观察组患者总有效率为93.18%，明显高于对照组的77.78%（P<0.05）。治疗后，观察组中医症状体征得分及抑郁自评量表得分均明显低于对照组、生活质量评分高于对照组（均P<0.05）；观察组患者单核细胞趋化因子-1（monocyte chemoattractant protein-1，MCP-1）、超敏C-反应蛋白（high-sensitrity c-reactive protein，hrity c-reactive protein，hs-CRP）、基质金属蛋白酶-9（matrix metalloprotein-9，MMP-9）均明显低于对照组（P<0.05）；治疗前后两组患者均未出现明显异常，对照组患者出现2例不良反应，观察组出现1例，两组差异无统计学意义（P>0.05）。结论 炙甘草汤联合常规西医治疗较单独西医治疗可明显提高慢性心衰合并抑郁症的临床疗效，明显改善中医症状体征，提高生活质量，缓解抑郁状态，降低MCP-1、hs-CRP及MMP-9水平，且无明显不良反应。 |

| Clinical Observation on the Treatment of Chronic Heart Failure with Depression by Zhigancao Decoction Combined with Routine Western Medicines |

| Objective To explore the clinical efficacy of Zhigancao Decoction combined with routine Western medicines in the treatment of chronic heart failure with depression. Methods A totally of 89 patients with chronic heart failure and depression who were treated in our hospital from March 2017 to March 2019 were studied. According to the different treatments of patients, they were divided into 2 groups. 45 patients in the control group were treated with conventional western medicine, and 44 patients in the observation group were treated with Zhigancao Decoction on the basis of conventional western medicine. The clinical efficacy, traditional Chinese medicine (TCM) symptoms and signs, quality of life, depression, biochemical indicators and safety indicators of the 2 groups were compared. Results The total effective rate of the observation group was 93.18%, which was significantly higher than 77.78% of the control group (P<0.05). After treatment, the scores of TCM symptoms and signs, quality of life and self-rating depression scale in the control group were significantly lower than those in the observation group (all P<0.05); MCP-1, hs-CRP and MMP-9 in the control group were significantly lower than those in the observation group (P<0.05). There were no obvious abnormalities in both groups before and after treatment. There were 2 cases of adverse reactions in the control group and 1 case in the observation group. There was no significant difference between the 2 groups (P>0.05). Conclusion Zhigancao Decoction combined with conventional western medicine treatment can significantly improve the clinical efficacy of chronic heart failure combined with depression, significantly improve the symptoms and signs of traditional Chinese medicine. It also can improve the quality of life, alleviate depression, and reduce the levels of MCP-1, hs-CRP and MMP-9, without obvious adverse reactions. |
