| 中药汤剂联合西药治疗老年气滞血瘀型冠心病心绞痛患者的临床观察 |

| 摘要: |
| 目的 观察自拟通脉益气活血汤联合常规西药治疗老年气滞血瘀型冠心病心绞痛患者的临床疗效。方法 选取本院2017年1月至2018年6月就诊并住院治疗的冠心病心绞痛患者72例，随机分为观察组和对照组（各36例）。对照组采用常规西药治疗，观察组在对照组基础上采用自拟通脉益气活血汤，均治疗1个月。观察比较两组患者治疗前后的心绞痛发作频率、发作的持续时间、血脂和血液流变学相关指标[血清总胆固醇（TC）、甘油三酯（TG）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、血细胞比容（HCT）及血小板黏附率（PADT）]，治疗后两组患者的临床疗效和不良反应。结果 （1）观察组总有效率高于对照组，差异有统计学意义（P<0.05）；（2）两组患者治疗后心绞痛发作频率和发作持续时间均较治疗前降低（P<0.05），且治疗后观察组较对照组低（P<0.05）；（3）两组患者治疗后较治疗前TC、TG、LDL-C降低，HDL-C含量升高，血液流变学指标HCT、PADT降低（P<0.05）；且治疗后观察组TC、TG、LDL-C含量均较对照组低，HDL-C含量较对照组高，HCT、PADT均明显低于对照组（P<0.05）；（4）两组患者未见严重不良反应，其总发生率比较差异无统计学意义（P>0.05）。结论 自拟通脉益气活血汤联合常规西药治疗老年气滞血瘀型冠心病心绞痛患者疗效显著，且优于单纯西药治疗，值得临床推广。 |

| Clinical Effect of Traditional Chinese Medicine Decoction Combined with Western Medicine Treatment in Elderly Patients with Angina Pectoris with Qi Stagnation and Blood Stasis |

| Objective To investigate the clinical effect of Tongmai Yiqi Huoxue Decoction, a self-made traditional Chinese medicine decoction, combined with conventional Western medicine treatment in elderly coronary heart disease (CHD) patients with angina pectoris with Qi stagnation and blood stasis.Methods A total of 72 CHD patients with angina pectoris who were hospitalized and treated in our hospital from January 2017 to June 2018 were enrolled and randomly divided into observation group and control group, with 36 patients in each group. The patients in the control group were given conventional Western medicine treatment, and those in the observation group were given Tongmai Yiqi Huoxue Decoction in addition to the treatment in the control group. The course of treatment was one month for both groups. The two groups were compared in terms of the frequency and duration of angina pectoris attack and blood lipids and hemorheological indices[total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), hematocrit (HCT), and platelet adhesion rate (PADT)] before and after treatment, as well as clinical outcome and adverse reactions after treatment.Results The observation group had a significantly higher overall response rate than the control group (P<0.05). After treatment, both groups had significant reductions in the frequency and duration of angina pectoris attack (P<0.05), and the observation group had a significantly lower frequency and a significantly shorter duration (P<0.05). After treatment, both groups had significant reductions in TC, TG, and LDL-C, a significant increase in HDL-C, and significant reductions in HCT and PADT (P<0.05), and compared with the control group, the observation group had significantly lower TC, TG, and LDL-C, a significantly higher HDL-C level, and significantly lower HCT and PADT (P<0.05). No serious adverse reactions were observed in either group, and there was no significant difference in overall incidence rate between the two groups (P>0.05).Conclusion Tongmai Yiqi Huoxue Decoction combined with conventional Western medicine treatment has a good clinical effect in elderly CHD patients with angina pectoris with Qi stagnation and blood stasis and is better than Western medicine alone. Therefore, it holds promise for clinical application. |
