| 目的 探讨中药外敷联合滑膜炎颗粒对膝关节急性滑膜炎的疗效及红细胞沉积率（ESR）、白细胞介素-1（IL-1）、肿瘤坏死因子-α（TNF-α）的影响。方法 选择2014年11月至2016年11月本院接诊的92例膝关节急性滑膜炎患者，按随机数字表法分为观察组和对照组各46例，对照组给予滑膜炎颗粒口服，观察组联合自拟中药方外敷。两组均以10 d为1个疗程，连续治疗3个疗程。比较两组治疗前后ESR、IL-1、TNF-α及症状积分的变化，并比较两组疼痛、肿胀缓解时间、临床疗效及不良反应。结果 治疗后，两组ESR、IL-1、TNF-α和疼痛、肿胀、活动度积分均较治疗前显著降低（P<0.05），且观察组低于对照组（P<0.05）；观察组疼痛、肿胀缓解时间均明显短于对照组（P<0.05）；观察组临床疗效总有效率明显高于对照组（P<0.05）；两组治疗过程中均无明显不良反应发生。结论 膝关节急性滑膜炎患者应用中药外敷联合滑膜炎颗粒治疗效果显著，可有效缓解关节疼痛、肿胀，改善关节活动度，其机制可能与调节ESR、IL-1、TNF-α的表达相关。 |

| Objective To investigate the clinical effect of external application of traditional Chinese medicine combined with Huamoyan Granule in the treatment of acute synovitis of the knee joint and its influence on erythrocyte sedimentation rate (ESR), interleukin-1 (IL-1), and tumor necrosis factor-α (TNF-α). Methods A total of 92 patients with acute synovitis of the knee joint who visited our hospital from November 2014 to November 2016 were enrolled and divided into observation group and control group using a random number table, with 46 patients in each group. The patients in the control group were given oral Huamoyan Granule, and those in the observation group were given external application of self-made traditional Chinese medicine prescription in addition to the treatment in the control group. Each course of treatment was 10 days, and both groups were treated for 3 consecutive courses. The two groups were compared in terms of the changes in ESR, IL-1, TNF-α, and symptom score after treatment, time to relief of pain and swelling, clinical outcome, and adverse events. Results After treatment, both groups had significant reductions in ESR, IL-1, TNF-α, and scores of pain, swelling, and range of motion (P<0.05), and the observation group had significantly lower values than the control group (P<0.05). The observation group had a significantly shorter time to relief of pain and swelling than the control group (P<0.05). The observation group had a significantly higher overall response rate than the control group (P<0.05). No adverse events were observed during treatment. Conclusion In patients with acute synovitis of the knee joint, external application of traditional Chinese medicine combined with Huamoyan Granule has a marked clinical effect and can improve joint pain, swelling, and range of motion, possibly by regulating the expression of ESR, IL-1, and TNF-α. |
