| 慢性肝衰竭患者虹膜特征与中医证型相关性研究 |

| 摘要: |
| 目的 探讨慢性肝衰竭患者虹膜特征与中医证型的相关性。方法 以符合纳入标准的慢性肝衰竭患者150例，50例体检正常者作为对照，使用虹膜诊断仪采集观察对象虹膜图像用于虹膜特征性分析。按照Jensen氏虹膜定位理论，利用CV Advance Analysis System，从右眼虹膜肝脏分区及双眼虹膜全局上坑洞、斑块、裂缝等改变作为观察指标。分析虹膜特征与证型之间的相关性；建立虹膜诊断证型判别模型；基于虹膜诊断理论及总结的虹膜特征与中医证型相关性、CV Advance Analysis System，虹膜诊断证型判别模型，采集慢性肝衰竭患者虹膜图像作出证型判别，并与临床医师的中医辨证进行一致性分析。结果 （1）慢性肝衰竭患者右眼虹膜肝脏分区，均无特征性改变，说明慢性肝衰竭患者在虹膜肝脏分区不存在相关性。（2）慢性肝衰竭患者阳黄组、阴黄组、阴阳黄组3个证型的虹膜特征与对照组比较，差异有显著统计学意义（P＜0.01）。（3）阳黄证与坑洞、裂缝存在相关性，与斑块不存在相关性，与裂缝这一特征性改变的相关性具有显著性（P＜0.01）。阴黄证与斑块、裂缝存在相关性，与坑洞的相关性具有显著性（P＜0.01）。阴阳黄证与坑洞、斑块、裂缝均存在相关性，但与斑块的相关性具有显著性（P＜0.01）。（4）建立的虹膜诊断判别模型，训练样本回代法示阳黄证判别符合率66.7%，阴黄证为60.0%，阴阳黄证为70.0%。3种证型判别的总符合率为65.6%。（5）虹膜特征证型判别与临床医师中医辨证的证型具有一致性（P＜0.05）。结论 虹膜诊断对慢性肝衰竭中医证型有意义。 |

| Objective To investigate the correlations between TCM syndrome and iris features of chronic liver failure patients. Methods 150 cases of patients meet the inclusion criteria of chronic liver failure were selected. 50 cases of healthy people as the controls, the iris features were analyzed by the collected iris images from iris diagnostic apparatus. The changes of liver partitions in iris of right eyes and potholes, plaque, crack in two eyes were as the observation indicators with CV Advance Analysis System. The correlations between the iris feature and TCM syndrome types were analyzed. The discrimination models of iris diagnosis were built. The iris images of chronic liver failure patients were as discrimination pattern of syndrome based on theory, iris feature and TCM syndrome type correlations, CV Advance Analysis System, and discrimination models of iris diagnosis, and were made the consistency analysis with TCM syndrome by clinicians. Results (1) The right eye iris liver partitions in patients with chronic liver failure have no characteristic changes; there is no correlation between patients with chronic liver failure and the iris liver partition. (2) Compared with the control group, the differences of iris features of chronic liver failure patients in Yang Huang group, Yin Huang group and Yin-Yang Huang group have statistical significance (P<0.01). (3) The Yang Huang syndrome, has correlations with cracks and potholes, no correlation with plaque, have a significant correlation with the characteristic change of cracks (P<0.01); there was a correlation between the Yin Huang syndrome and plaque, cracks, holes, have a significant correlation with potholes (P<0.01); Yin-Yang Huang syndrome has correlations with potholes, plaque and cracks, have a significant correlation with plaque (P<0.01). (4) The Yang Huang syndrome discrimination coincidence rate 66.7%, Yin Huang coincidence rate 60%, Yin-Yang Huang syndrome coincidence rate of 70%. Three types of discrimination of the total coincidence rate were 65.6%. (5) The iris image discriminant syndromes and syndrome differentiation of TCM show the consistency. Conclusion Iris diagnosis has significance to chronic liver failure in syndrome of TCM. |
