| 摘要: |
| 目的 探究益气定眩汤联合针灸治疗青年颈性眩晕的疗效及对血清神经肽Y（neuropeptide-Y, NPY）、尾加压素Ⅱ（urotensinⅡ, UⅡ）浓度的影响。方法 选取2020年1月至2021年10月沧州市中心医院收治的104例青年颈性眩晕患者作为研究对象，按照随机数字表法分为观察组、对照组，各52例。对照组采用针灸治疗，观察组在对照组基础上加用益气定眩汤治疗。比较2组疗效、治疗前后中医证候积分、血液流变学指标（血浆黏度、低切全血黏度、高切全血黏度）、椎-基底动脉血流动力学指标[搏动指数（pulseindex, PI）、平均血流速度（mean blood flow velocity, Vm）]、血清NPY及UⅡ水平、不良反应发生率。结果 观察组总有效率92.31%，高于对照组的78.85%（P＜0.05）。治疗后，2组患者中医证候积分、血浆黏度、低切全血黏度、高切全血黏度较治疗前低，椎动脉、基底动脉Vm较治疗前快，PI较治疗前慢，血清NPY、UⅡ水平较治疗前低，差异均有统计学意义（P＜0.05）。治疗后，观察组中医证候积分、血浆黏度、低切全血黏度、高切全血黏度较对照组低，椎动脉、基底动脉Vm较对照组快，PI较对照组慢，血清NPY、UⅡ水平较对照组低，差异均有统计学意义（P＜0.05）。2组不良反应发生率比较，差异无统计学意义（P>0.05）。结论 益气定眩汤联合针灸治疗青年颈性眩晕效果显著，可有效减轻临床症状，改善血液流变学及血流动力学指标，调节血清NPY、UⅡ水平，且安全性高。 |

| Objective To explore the efficacy of Yiqi Dingxuan Decoction combined with acupuncture in the treatment of cervical vertigo in young people and the effects on serum neuropeptide-Y (NPY) and urotensin II (UII) concentrations. Methods A total of 104 young patients with cervical vertigo admitted to Cangzhou Central Hospital from January 2020 to October 2021 were selected as the research objects and they were divided into observation group and control group according to the random number table method, with 52 cases in each group. The control group was treated with acupuncture, and the observation group was treated with Yiqi Dingxuan Decoction on the basis of the control group. The curative effect, TCM syndrome scores, hemorheology indexes [plasma viscosity, low shear whole blood viscosity, high shear whole blood viscosity], vertebrobasilar hemodynamics indexes [pulse index (PI), mean blood flow velocity, serum levels of NPY and UII, and incidence of adverse reactions were compared between two groups before and after treatment.. Results The total effective rate of the observation group was 92.31%, higher than that of the control group (78.85%, P＜0.05). After treatment, the TCM syndrome score, plasma viscosity, low shear whole blood viscosity and high shear whole blood viscosity of two groups were lower than those before treatment; Vm of vertebral artery and basilar artery was faster than those before treatment; PI was slower than those before treatment, and the levels of serum NPY and UII were lower than those before treatment, with statistical significance (P＜0.05). After treatment, the TCM syndrome score, plasma viscosity, low shear whole blood viscosity and high shear whole blood viscosity in the observation group were lower than those in the control group; Vm of vertebral artery and basilar artery was faster than that in the control group. PI was slower than that in the control group, and serum NPY and UII levels were lower than those in the control group, with statistical significance (P＜0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Yiqi Dingxuan Decoction combined with acupuncture shows significant efficacy in treating cervical vertigo of young patients, which can effectively relieve clinical symptoms, improve hemorheology and hemodynamic indexes, and regulate serum NPY and UII levels with high safety. |
