| 摘要: |
| 目的 观察在康复训练基础上采用自拟强腰汤、穴位按压对老年腰椎间盘突出症（lumbar disc herniation,LDH）患者经后路腰椎融合术后功能恢复及其生活质量的影响。方法 采用随机数字表法将本院2017年9月至2019年2月收治的180例LDH患者分为观察组和对照（各90例），对照组采用康复训练，观察组在对照组基础上采用自拟强腰汤、穴位按压疗法，观察治疗前后两组患者的腰部疼痛程度[视觉模拟评分法（visual analogue score,VAS）评分]、腰椎功能评分[Oswestry功能障碍指数(Oswestrydisability index,ODI)和日本骨科协会（Japanese orthopaedic association,JOA）评分]、腰椎关节活动度及日常生活活动能力量表（Barthel指数）变化，比较两组患者治疗后的临床疗效。结果 （1）治疗后，两组患者VAS、ODI评分均较治疗前降低，JOA评分较治疗前升高（P<0.05）；且观察组VAS、ODI评分低于对照组，JOA评分高于对照组（P<0.05）。（2）治疗后，两组患者腰椎前屈/后伸活动度及Barthel指数评分均较治疗前升高（P<0.05），且观察组腰椎前屈/后伸活动度、Barthel指数评分均高于对照组（P<0.05）。（3）观察组总有效率高于对照组（P<0.05）。结论 在康复训练基础上采用自拟强腰汤、穴位按压治疗老年LDH患者疗效显著，可显著减轻患者腰部疼痛、改善腰椎功能，并提高腰椎关节活动度和日常生活活动能力。 |

| Objective To observe the effect of self-made Qiangyao Decoction and acupoint pressing on the functional recovery and quality of life of elderly patients with lumbar disc herniation (LDH) after posterior lumbar fusion on the basis of rehabilitation training. Methods A total of 180 patients with LDH admitted to the hospital from September 2017 to February 2019 were divided into an observation group and a control group (90 cases in each group) according to random number table. Rehabilitation training was used in the control group. On the basis of the control group, self-made Qiangyao Decoction and acupoint pressing therapy were used in the observation group. The level of lumbar pain [visual analogue score (VAS)], lumbar spine function score [Oswestry disability index (ODI)] and Japanese Orthopaedic Association (JOA) score, changes in the degree of motion of the lumbar spine joints and the daily living activity scale (Barthel index) of the 2 groups before and after treatment were observed. The clinical efficacy of the 2 groups of patients after treatment was compared. Results (1) After treatment, the scores of VAS and ODI in the 2 groups were decreased than those before treatment, and the scores of JOA were increased than those before treatment (P<0.05). The scores of VAS and ODI in the observation group were lower than those in the control group, and the scores of JOA were higher than those in the control group (P<0.05). (2) After treatment, the scores of lumbar flexion / extension activity and Barthel Index in the 2 groups were increased than those before treatment (P<0.05), and the scores of lumbar flexion / extension activity and Barthel Index in the observation group were higher than those in the control group (P<0.05). (3) The total effective rate of the observation group was higher than that of the control group (P<0.05). Conclusion On the basis of rehabilitation training, the self-made Qiangyao Decoction and acupoint pressing in the treatment of elderly LDH patients can significantly reduce the lumbar pain, improve the lumbar function, and improve the activity of lumbar joint and the ability of daily life. |
