| 摘要: |
| 目的 探研补肾填精之益肾通癃汤对前列腺癌去势治疗的减毒增效作用。方法 将38例病理诊断为前列腺癌且经药物去势治疗3个月的患者随机分为观察组与对照组，每组19例。对照组采用醋酸戈舍瑞林缓释植入剂腹前壁皮下注射联合比卡鲁胺片口服治疗，观察组在此基础上增服益肾通癃汤。两组疗程均为4个月。观测治疗前后两组患者血清总前列腺特异性抗原（total prostate-specific antigen，tPSA）、国际前列腺症状评分（international packet switched service，IPSS）及中医证候评分变化。结果 两组治疗后的血清tPSA水平均较治疗前显著下降（P<0.05），但组间差异无统计学意义（P>0.05）。两组治疗后的IPSS均较治疗前降低（P<0.05），且组间差异有统计学意义（P<0.05）。观察组患者的中医证候评分在治疗后明显下降（P<0.05），且明显低于对照组（P<0.05）。结论 以补肾填精为治法的益肾通癃汤不仅能维持去势治疗降低PSA的临床疗效，还能进一步降低患者IPSS，更能明显减轻去势治疗所带来的不良反应，提高患者的生活质量。 |

| Objective To explore the reducing toxicity and increasing efficacy of Yishen Tonglong Decoction on castration therapy of prostate cancer. Methods A total of 38 patients with pathological diagnosis of prostate cancer and 3 months after castration were randomly assigned to an observation group and a control group, with 19 cases in each group. The control group was treated with subcutaneous injection of goserelin acetate sustained-release depot in the anterior wall of the abdomen. On the basis of this, the observation group was given Yishen Tonglong Decoction. The course of both treatments was 4 months. The serum total prostate-specific antigen (tPSA), international packet switched service (IPSS) and traditional Chinese medicine (TCM) syndrome scores were observed before and after treatment. Results After treatment, serum tPSA levels were significantly decreased in the 2 groups than before treatment (P<0.05), but the difference between the groups was not statistically significant (P>0.05). The IPSS after treatment was decreased than that before treatment (P<0.05), and the difference between the groups was statistically significant (P<0.05). The TCM syndrome scores of the observation group decreased significantly after treatment (P<0.05), and it was significantly lower than the control group (P<0.05). Conclusion With the treatment method of reinforcing kidney to replenish essence, Yishen Tonglong Decoction can not only maintain the clinical efficacy of castration therapy in reducing PSA, but also further reduce the patient's IPSS, and can significantly reduce the adverse reactions caused by castration therapy and improve patient's quality of life. |
